补肾调冲活血法联合激素补充治疗早发性卵巢功能不全的临床研究

摘　　要：目的探讨补肾调冲活血法联合激素补充治疗早发性卵巢功能不全(POI)的临床疗效。方法选取70例POI患者,根据患者治疗方法不同分为治疗组(39例)与对照组(31例)。对照组患者应用常规激素补充方式进行治疗,治疗组患者在对照组基础上联合应用补肾调冲活血法进行治疗。对比两组临床疗效、治疗前后月经改善情况、中医症状积分、激素水平及卵巢大小情况。结果治疗组的临床总有效率94.87%高于对照组的77.42%,差异具有统计学意义(P<0.05)。治疗3个周期后和停药3个月后,治疗组中医症状积分均低于对照组(P<0.05);治疗组的卵泡刺激素(FSH)、促黄体生成素(LH)、雌二醇(E_(2))及抗苗勒管激素(AMH)水平均优于对照组(P<0.05);治疗组的卵巢窦卵泡数(AFC)多于对照组,子宫内膜厚度(EM)、子宫体积、卵巢体积均大于对照组,差异均具有统计学意义(P<0.05)。结论补肾调冲活血法联合激素补充治疗POI患者疗效确切,可有效改善症状和激素水平,促进排卵,值得临床推广。Objective To explore the clinical effect of invigorating Bushen tiaochong huoxue therapy and activating blood combined with hormone supplement on early-onset ovarian insufficiency(POI).Methods 70 patients with POI were selected and divided into treatment group(39 cases)and control group(31 cases)according to different treatment methods.The control group was treated with conventional hormone supplement,and the treatment group was treated with Bushen tiaochong huoxue therapy and promoting blood circulation on the basis of the control group.The clinical efficacy,menstrual improvement,TCM symptom score,hormone level and ovary size were compared between the two groups.Results The total clinical effective rate of the treatment group was 94.87%higher than that of the control group 77.42%,and the differences were statistically significant(P<0.05).After 3 cycles of treatment and 3 months after withdrawal,the scores of TCM symptoms in the treatment group were lower than those in the control group(P<0.05).The levels of follicle stimulating hormone(FSH),luteinizing hormone(LH),estradiol(E_(2))and anti-mullerian hormone(AMH)in treatment group were better than those in control group(P<0.05).The number of antra follicle counting(AFC)in the treatment group was higher than that in the control group,and the endometrial thickness(EM),uterine volume and ovarian volume were higher than those in the control group,with statistical significance(P<0.05).Conclusion Bushen tiaochong huoxue therapy combined with hormone supplement has definite curative effect on PoI patients,can effectively improve symptoms and hormone levels,promote ovulation,and is worthy of clinical promotion.
