摘　　要：目的 探讨针刺通督醒神穴在中风后意识障碍患者中的促醒作用及对脑血流动力学、体感诱发电位的影响。方法 选取2018年10月至2021年10月收治的80例中风后意识障碍患者为研究对象,基于治疗方式的差异将其分为对照组和观察组,各40例。对照组采用常规治疗,观察组在常规治疗基础上联合针刺通督醒神穴治疗。比较两组的治疗效果。结果 治疗后,观察组的格拉斯哥昏迷量表(GCS)、修订版昏迷恢复量表(CRS-R)评分均高于对照组(P<0.05)。治疗后,观察组的基质细胞衍生因子-1(SDF-1)水平高于对照组,视锥蛋白样蛋白-1(VILIP-1)水平低于对照组(P<0.05)。治疗后,观察组的平均血流速度(V_(m))、平均血流量(Q_(m))高于对照组,脑血管外周阻力(R)低于对照组(P<0.05)。治疗后,观察组的潜伏期短于对照组,波幅高于对照组(P<0.05)。结论 针刺通督醒神穴用于中风后意识障碍患者中的促醒作用较好,可改善神经功能、脑血流动力学及体感诱发电位,值得推广。Objective To explore the wake-promoting effect of acupuncture on Tongdu Xingshen acupoint in patients with consciousness disorder after stroke and its influences on cerebral hemodynamics and somatosensory evoked potential.Methods A total of 80 patients with consciousness disorder after stroke admitted from October 2018 to October 2021 were selected as the research objects.Based on the difference of treatment methods,the patients were divided into control group and observation group,with 40 cases in each group.The control group was treated with routine treatment,and the observation group was treated with acupuncture on Tongdu Xingshen acupoint on the basis of routine treatment.The therapeutic effects of the two groups were compared.Results After treatment,the scores of Glasgow Coma Scale(GCS)and Coma Recovery Scale-Revised(CRS-R)in the observation group were higher than those in the control group(P<0.05).After treatment,the cell-derived factor-1(SDF-1)level in the observation group was higher than that in the control group,and the visinin-like protein-1(VILIP-1)level was lower than that in the control group(P<0.05).After treatment,the mean blood flow velocity(V_(m))and mean blood flow(Q_(m))in the observation group were higher than those in the control group,and the cerebrovascular peripheral resistance(R)was lower than that in the control group(P<0.05).After treatment,the incubation period of the observation group was shorter than that of the control group,and the amplitude was higher than that of the control group(P<0.05).Conclusion Acupuncture on Tongdu Xingshen acupoint has a good wake-promoting effect in patients with consciousness disorder after stroke.It can improve neurological function,cerebral hemodynamics and somatosensory evoked potential. It is worth promoting.
