经食管超声心动图探头多平面角度与探头朝向关系的研究

摘  要:目的:探讨经食管超声心动图(TEE)探头多平面角度与探头朝向的关系。方法:利用一例心脏结构正常患者的冠脉CT资料,在Mimics/3-matic软件中建立心脏模型,利用空间解析几何知识和计算机编程的辅助,还原TEE检查过程,用数学方法探讨探头多平面角度与探头朝向之间的关系。结果:TEE在77°与100°观察到前交界,相应探头朝向分别在AB_(1)^(-)和AB_(2)^(-)方向上。因AB_(1)^(-)与AB_(2)^(-)夹角很小,探头朝向发生从AB_(1)^(-)到AB_(2)^(-)的变化当时没有被觉察。TEE观察二尖瓣时,前交界可以在探头的朝向位置与多平面角度的多个组合中被观察到,当探头的朝向位置改变时,多平面角度也相应改变。探头朝向发生较小改变时,即可引起多平面角度较大改变。本例患者二维TEE之所以可分别在77°和100°看到前交界处的大量二尖瓣反流,是因探头朝向在检查过程中不经意间发生6°改变而造成。Objective The relationship between multiplane angle and probe orientation of transesophageal echocardiography(TEE)probe were explored.Methods We utilized the data of a 45-year-old male who complained of chest distress and underwent cardiac computed tomographic angiography for diagnostic purposes.Subsequently,we established a heart model using the Mimics/3-matic software.Based on knowledge of spatial analytic geometry and the assistance of computer programming,we simulated the TEE process in 3-matic which included acquiring the transducer location,the transducer direction,and the 77°and 100°TEE views.Results During the TEE process,the center of the transducer was placed at point A.If the transducer was toward AB_(1)^(-)with a multiplane transducer angle of 77°,AL was evaluated via 2D TEE.If it was toward AB_(2)^(-)at 100°,AL was assessed again via 2D TEE.The coordinates of B_(1) and B_(2) were extremely close to each other.When TEE is performed to observe a mitral valve,AL can be observed in many combinations of transducer directions and multiplane transducer angles.The multiplane transducer angle may change obviously with a slight change of the transducer direction.The phenomenon in the patient with MVP could be likely explained as:when TEE was performed,the transducer was turned for 6°without being realized.
