张力带技术在基层骨科骨折治疗中的临床效果分析

摘　　要：目的探讨空心拉力螺钉张力带与改良AO张力带内固定在基层骨科髌骨骨折中的疗效差异,以为基层骨科骨折治疗提供参考。方法纳入我院2020年1月至2022年12月80例髌骨骨折患者,依据治疗方法的不同分为对照组与观察组,对照组41例采用空心拉力螺钉张力带内固定治疗,观察组39例采用改良AO张力带内固定治疗。比较两组患者的临床效果差异,包括治疗总有效率、手术相关指标、膝关节功能恢复指标、术后疼痛评分、不良反应情况以及心理状态、患者治疗满意度、生活质量的差异。结果观察组的关节屈曲度、Lysholm膝关节评分、患者治疗满意度、生活质量评分显著高于对照组,差异有统计学意义(P<0.05);观察组的术后疼痛评分、HAMD评分、HAMA评分明显低于对照组(P<0.05)。观察组的手术时间、住院费用显著低于对照组(P<0.05)。但两组的治疗总有效率、不良反应发生率以及骨折愈合时间比较,差异无统计学意义(P>0.05)。结论改良AO张力带内固定与空心拉力螺钉张力带内固定治疗基层骨科髌骨骨折均疗效肯定,但改良AO张力带内固定具有手术时间短、操作简便、费用低等优点,较空心拉力螺钉张力带内固定具有明显优势,已成为基层医院骨科治疗髌骨骨折最为有效的张力带内固定方法。Objective To investigate the differences in the efficacy of cannulated lag screw tension band and modified AO tension band internal fixation in the treatment of patellar fracture in primary hospitals.Methods A total of 80 patients with patellar fracture in department of orthopedics in our hospital from January 2020 to December 2022 were enrolled in this study,and then divided into control group and observation group according to different treatment methods.41 patients in the control group were treated with cannulated lag screw tension band internal fixation,and 39 patients in the observation group were treated with modified AO tension band internal fixation.The clinical effects of the two groups were compared,including the total effective rate,surgical related indicators,knee function recovery indicators,postoperative pain score,adverse reactions,psychological status,patient satisfaction,and quality of life.Results The joint flexion,Lysholm knee score,patient satisfaction and quality of life score in the observation group were higher than those in the control group(P<0.05).The postoperative pain score,HAMD score and HAMA score in the observation group were lower than those in the control group(P<0.05).The operation time and hospitalization cost in the observation group were lower than those in the control group(P<0.05).However,there was no statistical difference in the total effective rate,incidence of adverse reactions and fracture healing time between the two groups(P>0.05).Conclusions Both modified AO tension band internal fixation and cannulated tension screw tension band internal fixation are effective in the treatment of patellar fracture in primary hospitals.However,compared with cannulated tension screw tension band internal fixation,modified AO tension band internal fixation has the advantages of short operation time,simple operation and low cost.
