摘 要:目的探讨基于儿童疼痛管理的护理在腹腔镜手术治疗先天性胆总管囊肿患儿中的价值。方法选取我院2019年12月至2021年12月收治的先天性胆总管囊肿患儿20例,采取信封法随机分为研究组与对照组,各10例。其中对照组采取传统术前患者教育模式,研究组在此基础上采用基于儿童疼痛管理的护理模式。比较两组患儿围手术期指标(术后活动时间、排便时间、腹腔引流管拔管时间、术后住院时间),采用患儿麻醉诱导期合作量表(ICC)评分比较两组患儿术前麻醉诱导配合度,采用Wong-Baker面部表情疼痛量表法比较两组患儿术后疼痛情况以及两组患儿术后1周内并发症发生情况。结果研究组患儿的术后活动时间、排便时间、腹腔引流管拔管时间、术后住院时间以及术后3、12、24 h的Wong-Baker面部表情疼痛量表评分均短于或低于对照组,差异均有统计学意义(P<0.05);研究组ICC评分高于对照组,差异有统计学意义(P<0.05)。两组患儿术后并发症发生率差异无统计学意义(P>0.05)。结论基于儿童疼痛管理的护理模式用于腹腔镜手术治疗先天性胆总管囊肿患儿,可以改善患儿术前麻醉诱导配合度,有助于患儿术后快速恢复,减轻患儿自觉术后疼痛感。Objective To explore the value of Teach-back education combined with mind map in the laparoscopy treatment of congenital choledochal cyst.Methods Twenty children with congenital choledochal cysts admitted to our hospital from December 2019 to December 2021 were randomly divided into a study group and a control group using the envelope method,with 10 cases in each group.The control group adopted the traditional preoperative patient education mode,and the research group adopted the nursing mode based on Pain management in children on this basis.Compare the perioperative indicators(postoperative activity time,defecation time,abdominal drainage tube extubation time,and hospitalization time)of two groups of children.Use the Children's Anesthesia Induction Cooperation Scale(ICC)score to compare the preoperative anesthesia induction cooperation between the two groups of children.Use the Wong Baker Facial Expression Pain Scale method to compare the postoperative pain situation of the two groups of children and the incidence of postoperative complications within one week after surgery.Results The postoperative activity time,defecation time,abdominal drainage tube removal time,hospitalization time,and the Wong Baker facial expression pain scale scores at 3,12,and 24 hours after surgery in the study group were all lower than those in the control group,and the differences were statistically significant(P<0.05).The awareness rate of health education in the study group was higher than that in the control group(P<0.05).There was no significant difference in postoperative complications between the two groups(P>0.05).Conclusions Teach-back education combined with mind map can be used in the laparoscopy treatment of congenital choledochal cyst,which can improve the efficiency of operation and recovery,and reduce the bad mood of parents.
