淄博市儿童危重症救治单元发展情况调查分析

摘　　要：目的了解淄博市二级及以上医院儿科危重症救治单元建设现状,为淄博市儿童危重症救治单元的科学发展提供参考。方法采用网络问卷的形式,分别收集淄博市2020、2022年27家二级及以上医疗机构儿科危重症救治单元的资源配置数据,进行比较分析。结果本次淄博市有27家医院参与调查,有独立儿童重症医学科(pediatric intensive care unit,PICU)设置的单位3家,较2020年增加1家,两年总建筑面积、PICU床位数、PICU床位/儿科总床位、PICU医师人数、PICU护士人数均有不同程度增加,医床比、护床比有所下降,提示医护人员增幅小于床位增加数量。疾病转归,按儿童危重症病例评分(评分越低,病情越重)分组,>90分的人均住院日增加;80~90分的人均住院费用降低;<80分的人均住院日减少。各PICU单位对心包穿刺技术、持续血液净化技术、床旁临时心脏起搏技术应用率较低。2022年各单位抢救危重症患者较2020年增加21.6%。危重症病种分布,两年均以呼吸系统疾病为主。结论淄博市儿童危重症救治单元配置较合理,在人员配备、设备配备等方面较前有所发展,但危重症救治技术还需进一步提高完善,危重症患儿以呼吸系统疾病为主。Objective To investigate the current situation of construction of pediatrics critical care unit in secondary and above hospitals in Zibo.Methods A web-based questionnaire was used to collect data on the resource allocation of pediatric critical care units in 27 secondary and above medical institutions in Zibo in 2020 and 2022,respectively,for comparative analysis.Results There are 27 hospitals participating in this survey in Zibo,and there are 3 units with independent pediatric intensive care unit(PICU)in 2022,an increase of 1 unit compared with that in 2020,and the total floor area,number of PICU beds,number of PICU beds/total pediatric beds,number of PICU doctors,number of PICU nurses in 2022 increased in different degree compared to those in 2020,and the medical bed ratio and nursing bed ratio decreased,suggesting that the increase in medical staff was smaller than the number of beds.Grouped by child critical illness score,compared to 2022,the following indexes in 2022:the per hospital stay increased in the group with the score>90;lower per capita hospitalization costs in the group with the score of 80~90;the number of patients increased in the group with the score<80,and the per hospital stay decreased in the group with the score<80.The rate of PICU units applying pericardial puncture,continuous blood purification and bedside temporary cardiac pacing was low.In 2022,the number of critical patients rescued in all units increased by 21.6%compared with 2020.The distribution of critical diseases was dominated by respiratory diseases in the two years.Conclusions The allocation of children's critical illness treatment unit in Zibo City is more reasonable,and the personnel and equipment developed than before,but the critical illness treatment technology needs to be further improved.Most of the critical children suffered from respiratory diseases.
