螺旋CT在胃肠道间质瘤中的诊断价值

摘　　要：目的评价螺旋CT在胃肠道间质瘤中的诊断价值。方法纳入2019年1月~2022年1月期间在我院诊断疑似胃肠道间质瘤的患者104例,根据应用不同CT检查方式分为常规CT组和螺旋CT组,每组各52例。所有患者最后行病理学检查,并以病理检验结果作为金标准,分析不同检查方法对胃肠道间质瘤诊断价值。比较两组疾病检出率、诊断特异性、敏感度、准确性、检查持续时间、检查结果获得时间。结果经病理检查发现常规CT组42例(80.77%)确诊为胃肠道间质瘤,10例为其他胃肠道疾病,常规CT检出33例(63.46%)患者;螺旋CT组经病理确诊42例(80.77%)为胃肠道间质瘤,10例为其他胃肠道疾病,螺旋CT检出40例(76.92%)患者,螺旋CT组疾病检出率更高(P=0.038)。螺旋CT组诊断特异度、敏感度、准确性均高于常规CT组(90%vs.70%,95.24%vs.78.57%,94.23%vs.76.92%,均P=0.001)。螺旋CT组平均检查持续时间、平均检查结果获得时间均短于常规CT组[(3.26±0.63)min vs.(6.23±2.02)min,(3.02±0.45)h vs.(5.15±1.13)h,均P=0.001]。结论相比常规CT,螺旋CT诊断准确性、敏感度、特异度均较高,且检查时间与检查结果获得时间均较短,胃肠道间质瘤诊断价值更佳。Objective To evaluate the diagnostic value of spiral CT in gastrointestinal stromal tumors.Methods A total of 104 patients with suspected gastrointestinal mesenchymal tumors in our hospital from January 2019 to January 2022 were included.Based on different CT scan methods,all patients were divided into conventional CT group and spiral CT group,with 52 cases in each group.After CT scan,all patients were finally diagnosed by pathological examination,and the pathological results were used as the diagnostic gold standard.The detection rate,diagnostic specificity,sensitivity,accuracy,examination duration,and time of obtaining examination results of the two groups.Results In conventional CT group:42 cases(80.77%)of were diagnosed as gastrointestinal mesenchymal tumors by pathological examination,10 cases were other gastrointestinal diseases,33 cases(63.46%)were detected by conventional CT;in spiral CT group:42 cases(80.77%)were diagnosed as gastrointestinal mesenchymal tumors by pathological examination,10 cases were other gastrointestinal diseases,and 40 cases(76.92%)were detected by spiral CT.The detection rate was higher in the spiral CT group(P=0.038).The diagnostic specificity,sensitivity and accuracy of spiral CT group were all higher than those of conventional CT group(90%vs.70%,95.24%vs.78.57%,94.23%vs.76.92%,all P=0.001).The average examination duration and average time to obtain the results were shorter in spiral CT group than in conventional CT group[(3.26±0.63)min vs.(6.23±2.02)min,(3.02±0.45)h vs.(5.15±1.13)h,all P=0.001].Conclusions Compared with conventional CT,spiral CT has higher diagnostic accuracy,sensitivity and specificity,and the examination time and the time to obtain the results are shorter,with better diagnostic value for gastrointestinal mesenchymal tumors.
