摘 要:目的探讨医用单向阀在连续性肾脏替代治疗患者中静脉端钙补充中的应用。方法选取2021年1月至2021年8月使用枸橼酸抗凝的连续性肾脏替代治疗60例患者为对照组,在静脉端应用微量泵进行钙补充时,按常规方法在微量泵延长管端直接接入三通开关;另选取2021年9月至2022年7月使用枸橼酸抗凝的连续性肾脏替代治疗60例患者为观察组,在微量泵延长管接头与三通开关之间加入医用单向阀,观察两组患者每次续泵时延长管回血及更换延迟管时三通开关与延长管衔接处血液污染情况。结果观察组续泵时延长管回血率及更换延迟管时三通开关与延长管衔接处血液情况均显著低于对照组,两组比较差异有统计学意义(P<0.05)。结论医用单向阀应用于连续性肾脏替代治疗静脉端钙补充中,可有效减少续泵时微量泵延长管的回血、降低更换延长管时三通开关与延长管衔接处血液污染,降低血栓形成及导管相关性感染的风险。Objective To explore the application method of medical one-way valves in venous calcium supplementation in continuous renal replacement therapy patients.Methods Sixty patients who received continuous renal replacement therapy with citrate anticoagulation from January 2021 to August 2021 were selected as the control group.When using a micropump for calcium supplementation at the venous end,a three-way switch was directly connected to the extended end of the micropump using the conventional method.Another 60 patients who received continuous renal replacement therapy with citrate anticoagulation from September 2021 to July 2022 were selected as the observation group.A medical one-way valve was added between the extension tube joint of the micropump and the three-way switch to observe the blood contamination at the junction of the three-way switch and the extension tube during each pump extension and replacement of the delayed tube in both groups of patients.Results The blood return rate of the extended tube during pump continuation and the blood condition at the junction between the three-way switch and the extended tube during replacement of the delayed tube in the observation group were significantly lower than those in the control group,and the difference between the two groups was statistically significant(P<0.05).Conclusions The use of medical one-way valves in continuous renal replacement therapy for venous calcium supplementation can effectively reduce the return of blood to the micropump extension tube during pump continuation,reduce blood contamination at the junction of the three-way switch and extension tube when replacing the extension tube,and reduce the risk of thrombosis and catheter-related infections.
