系统护理对腹腔镜阑尾切除术患者术后肠胃功能恢复的影响

摘　　要：目的研究腹腔镜阑尾切除术患者在开展护理过程中实施系统护理的效果。方法纳入2021年10月至2023年8月我院选择腹腔镜阑尾切除术患者,随机分为试验组、对照组,对照组实施常规护理,试验组实施系统护理,比较两组术后胃肠功能恢复情况、术后疼痛改善程度、护理前后心理状态(HAMA、HAMD)、并发症发生率及护理前后睡眠质量(PSQI)情况。结果共纳入94例患者,每组47例。①试验组肠鸣音消失时间、肛门首次排气时间、排便时间、术后疼痛得分均优于对照组,组间对比(P<0.05)。②HAMA评分、HAMD评分,护理前,两组数据无统计学差异(P>0.05);护理后,试验组两项得分均低于对照组(P<0.05)。③试验组总并发症率低于对照组(P<0.05)。④睡眠质量,护理前,两组无统计学差异(P>0.05);护理后,试验组各项评分指数结果优于对照组(P<0.05)。结论腹腔镜阑尾切除术患者开展系统护理,更有利于患者术后胃肠功能恢复,减少负面情绪和更好缓解疼痛,并发症更少,预后更好。Objective To study the effects of systematic nursing on postoperative gastrointestinal function recovery in patients undergoing laparoscopic Appendectomy during the process of nursing care,as well as the impact on.Methods The inpatients with laparoscopic appendectomy who admitted in our hospital from October 2021 to August 2023 were included in the study.They were randomly divided into the observation group and control group,the control group was given routine nursing and the observation group was given systematic nursing.The recovery of gastrointestinal function,the improvement of pain,psychological status(HAMD,HAMA),the rate of complications,and sleep quality(PSQI score)after surgery were compared between the two groups.Results A total of 94 cases were included,with 47 in each group.(1)The disappearance time of bowel sounds,first anal exhaust time,defecation time,and postoperative pain score in observation group were all better than those in the control group(all P<0.05).(2)There was no difference in the HAMA score and HAMD score results between the two groups before nursing(P>0.05);After nursing,the two scores in the observation group were lower than those in the control group(both P<0.05);(3)The total rate of complications in the observation group was lower than that of the control group(P<0.05);(4)For sleep quality evaluation,there was no difference between the two groups before nursing(P>0.05);After nursing,the score in observation group was lower than that in the control group(P<0.05).Conclusions Compare to routine nursing,systematic nursing care for patients undergoing laparoscopic appendectomy can better promote the recovery of gastrointestinal function,relieve negative emotions and pain,improve the rehabilitation,reduce the rate of complications,and improve the patients'prognosis.
