摘　　要：目的研究鼻内镜手术联合布地奈德治疗慢性鼻窦炎对黏膜组织重塑及并发症的影响。方法选取2021年9月至2022年8月梅州市人民医院收治的90例慢性鼻窦炎患者,按随机数字表法分为对照组与观察组,每组各45例。两组均行鼻内镜手术治疗,对照组术后予以抗感染、止血等常规治疗,观察组在其基础上联合布地奈德治疗。比较两组患者治疗前后的治疗总有效率、Lund-Kennedy内镜评分、炎症因子[C反应蛋白(CRP)、白介素-6(IL-6)及肿瘤坏死因子-α(TNF-α)]水平、嗅觉功能、并发症发生情况。结果观察组患者的治疗总有效率为93.33%,高于对照组的75.56%,差异有统计学意义(P<0.05);观察组患者治疗后的Lund-Kennedy评分、炎症因子水平均低于对照组,嗅觉功能各项评分均高于对照组,差异有统计学意义(P<0.05);观察组患者的并发症总发生率低于对照组,差异有统计学意义(P<0.05)。结论鼻内镜手术联合布地奈德治疗慢性鼻窦炎效果显著,能够提高治疗总有效率,促进黏膜组织转归,降低并发症发生可能性,值得推广。Objective To study the effect of endoscopic sinus surgery combined with Budesonide on mucosal tissue outcome and complications in patients with chronic rhinosinusitis.Methods A total of 90 patients with chronic sinusitis admitted to Meizhou People's Hospital from September 2021 to August 2022 were selected and divided into control group and observation group according to random number table method,with 45 cases in each group.Both groups were treated with nasal endoscopic surgery.The control group was given conventional treatment such as anti-infection and hemostasis,and the observation group was treated with Budesonide on the basis of the control group.The total response rate,Lund-Kennedy endoscopic score,levels of inflammatory factors(C-reactive protein[CRP],interleukin-6[IL-6]and tumor necrosis factor-α[TNF-α]),olfactory function,and incidence of complications were compared between the two groups before and after treatment.Results The total effective rate of the observation group was 93.33%,which was higher than 75.56%of the control group,and the difference was statistically significant(P<0.05).After treatment,the Lund-Kennedy score and inflammatory factor levels of the observation group were lower than those of the control group,and the scores of olfactory function were higher than those of the control group,and the differences were statistically significant(P<0.05).The total incidence of complications in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Endoscopic sinus surgery combined with Budesonide in the treatment of chronic sinusitis has significant effect,which can improve the total effective rate of treatment,promote the outcome of mucosal tissue,and reduce the possibility of complications,which is worthy of promotion.
