摘  要:目的研究血清唾液酸,C3在支原体肺炎合并气道黏液栓患儿中的表达及临床意义。方法选取自2017年3月—2018年3月期间于本院诊治的206例支原体肺炎(MPP)患儿作为研究对象。根据支气管镜检查结果,将MPP患儿分为单纯MPP组130例和MPP伴黏液栓组76例。检测各组血清唾液酸和补体C3水平,统计学分析比较各组血清唾液酸和C3水平差异。受试者工作曲线分析(ROC)分析血清唾液酸和C3水平对MPP合并气道黏液栓的诊断价值。单因素及多因素COX回归分析影响支原体肺炎合并气道黏液栓形成的危险因素。结果相比于单纯MPP组,治疗前MPP合并气道黏液栓组血清唾液酸和C3水平显著较高,差异具有统计学意义(P<0.05)。ROC曲线分析结果,血清唾液酸、C3及联合检测的AUC分别为0.705、0.631及0.786,联合检测对MPP伴气道黏液栓诊断价值明显高于单个指标(P<0.05)。单因素及多因素COX回归分析发现,年龄>5岁、肺不张、血清唾液酸>86.23 mg/L和血清C3>1.35 g/L是MPP患儿气道黏液栓形成的独立危险因素。结论MPP合并气道黏液栓患儿血清唾液酸及补体C3水平升高,年龄>5岁、肺不张、血清唾液酸>86.23 mg/L和血清C3>1.35 g/L是MPP伴气道黏液栓形成的独立危险因素,检测血清血清唾液酸及补体C3水平有助于MPP合并气道黏液栓的早期诊断。Objective To study the expression and clinical significance of serum sialic acid and C3 in children with mycoplasma pneumonia and airway mucus plug.Methods 206 children with mycoplasma pneumonia(MPP)who were diagnosed and treated in our hospital from March 2017 to March 2018 were selected as the research objects.According to the results of bronchoscopy,children with MPP were divided into MPP alone group(130 cases)and MPP with mucus suppository group(76 cases).The levels of serum sialic acid and complement C3 in each group were detected,and the differences in serum sialic acid and C3 levels in each group were statistically analyzed and compared.Receiver operating curve analysis(ROC)were used to analyze the value of serum sialic acid and C3 levels in the diagnosis of MPP combined with airway mucus plugs.Univariate and multivariate COX regression analysis were used to analyze the influencing risk factors of mycoplasma pneumonia complicated with airway mucus embolism.Results Compared with the simple MPP group,the serum sialic acid and C3 levels of the MPP combined with airway mucus suppository group were significantly higher before treatment,and the difference was statistically significant(P<0.05).ROC curve analysis results show that serum sialic acid,C3 and the combined detection of AUC were 0.705,0.631 and 0.786,respectively.The combined detection of MPP with airway mucus plugs has a significantly higher diagnostic value than a single index(P<0.05).Univariate and multivariate COX regression analysis found that age>5 years old,atelectasis,serum sialic acid>86.23mg/L and serum C3>1.35g/L are independent risk factors for airway mucus thrombosis in children with MPP.Conclusion The levels of serum sialic acid and complement C3 in children with MPP complicated with airway mucus embolism are elevated,age>5 years old,atelectasis,serum sialic acid>86.23 mg/L and serum C3>1.35 g/L are independent risk factors for MPP with airway mucus,the detection of serum sialic acid and complement C3 levels is helpful for the early diagno
