摘 要:目的探讨葡萄糖酸氯己定皮肤消毒液联合改良型医用弹力网套在神经外科小儿外周静脉输液中的应用效果。方法选择温州市某三级甲等医院神经外科行外周静脉置管的患儿为研究对象,按入院时间先后分为3组,分别为聚维酮碘+传统型自黏弹力绷带组、葡萄糖酸氯己定+传统型自黏弹力绷带组和葡萄糖酸氯己定+改良型医用弹力网套组,每组60例,比较消毒待干时间、导管留置时间、导管局部不良情况和导管相关并发症。结果与聚维酮碘+传统型自黏弹力绷带组和葡萄糖酸氯己定+传统型自黏弹力绷带组相比,葡萄糖酸氯己定+改良型医用弹力网套组患儿的消毒待干时间最短,为(18.10±2.97)s;导管留置时间最长,为(86.95±29.15)h;发生敷贴卷边和导管拔出后胶布残留等导管局部不良情况例数最少(P<0.05)。3组导管相关并发症发生例数差异不显著(P>0.05)。结论葡萄糖酸氯己定皮肤消毒液联合改良型医用弹力网套可缩短消毒待干时间,延长导管留置时间,减少导管局部不良情况的发生。Objective To investigate effect of chlorhexidine gluconate skin disinfectant combined with modified elastic mesh for peripheral venous infusion of pediatric patients in neurosurgery.Methods Pediatric patients with peripheral venous infusion in neurosurgery of a class A tertiary hospital in Wenzhou were selected as the study subjects.According to the time of admission,they were divided into three groups:povidone-iodine+traditional self-adhesive elastic bandage group,chlorhexidine gluconate traditional+self-adhesive elastic bandage group,and chlorhexidine gluconate+modified elastic mesh group,with 60 cases in each group.The disinfection waiting time,catheter indwelling time,local adverse conditions of catheter and catheter-related complications were compared.Results Compared with the povidone-iodine+traditional self-adhesive elastic bandage group and the chlorhexidine gluconate+traditional self-adhesive elastic bandage group,the children in chlorhexidine gluconate+modified elastic mesh group had the shortest disinfection waiting time(18.10±2.97)s,and the longest catheter indwelling time(86.95±29.15)h.The occurrence of local adverse such as curling and adhesive tape residue after catheter removal was significantly reduced(P<0.05).The number of catheter-related complications in 3 groups were not significant(P>0.05).Conclusion Chlorhexidine gluconate skin disinfectant combined with modified elastic mesh can shorten the waiting time for disinfection,prolong the indwelling time of catheter and reduce the occurrence of local adverse conditions.
