摘　　要：目的总结乳腺腺瘤的分类和包括可疑恶性超声表现在内的超声特征,以提高对本疾病的认识。方法回顾性分析经病理证实的45例乳腺腺瘤患者的临床病理资料和超声特征。结果患者年龄17~55岁,平均年龄(28.68±6.53)岁;肿块大小8~50 mm,平均大小(21.53±4.21)mm。共有41例管状腺瘤、3例泌乳腺瘤、1例导管腺瘤,其中28例管状腺瘤和2例泌乳腺瘤表现为形态规则、边缘光整、均质回声、后方回声增强或无改变的良性超声特征,13例管状腺瘤和1例泌乳腺瘤表现为形态不规则、边缘模糊或分叶、不均质或混合回声的可疑恶性超声征象,1例导管腺瘤表现为扩张导管内实性低回声结节伴钙化。结论乳腺腺瘤大部分表现为与纤维腺瘤类似的良性肿瘤超声特征,约30%有可疑恶性征象,无典型恶性肿瘤征象。Objective To analyze the classification and the sonographic features of breast adenoma,including those suspected to be malignant,and to improve the understanding of this disease.Methods The clinical and pathological data and ultrasonic features of 45cases of breast adenoma confirmed by pathology in our hospital were retrospectively analyzed.Results The age of the patients was 17-55years old,with an average of(28.68±6.53)years old;the size of the mass was 8-50mm,with an average of(21.53±4.21)mm.There were 41cases of tubular adenoma,3cases of secretory breast tumor and one case of ductal adenoma.Among these cases,twenty-eight cases of tubular adenoma and two cases of lactating adenoma were presented as regular shape,smooth edge,homogeneous echogenicity,and exist or lack of posterior enhancement,which were considered as benign ultrasonic features.13cases of tubular adenoma and one case of lactating adenoma were suspected to be malignant with the ultrasonic signs of irregular shape,blurred edge or lobulated,heterogeneous or mixed echogenicity.One case of ductal adenoma showed solid hypoechoic nodule with calcification in the dilated duct.Conclusions Most of breast adenomas have similar benign ultrasonographic features of fibroadenoma.About 30%of breast adenomas have signs to be suspected as malignant,but without typical malignant characteristic.
