摘 要:目的探讨肾脏叶间动脉阻力指数(RI)和肾实质弹性成像预测1型糖尿病肾病的价值。方法选取90例1型糖尿病患者(研究组)和60例健康志愿者(对照组)进行右肾叶间动脉阻力指数和肾实质弹性测定,检测两组受试者的空腹血糖、糖化血红蛋白(HbA1c)、24 h尿蛋白、血肌酐等生化指标,测量血压,然后进行比较,判定两组差异是否有统计学意义。最后对研究组的肾叶间动脉RI、肾实质弹性与肾体积、血压及上述生化指标进行相关性分析。结果对照组与研究组比较,除血肌酐外,血压及所测生化指标差异均有统计学意义(P<0.05)。研究组肾体积增大,与对照组比较差异有统计学意义(P=0.000)。对照组肾实质平均剪切波速度为(2.91±0.32)m/s,研究组为(2.50±0.50)m/s(P=0.000)。研究组肾段动脉血流速度峰值、血流RI及叶间动脉血流RI较对照组高(P=0.000)。研究组肾实质硬度与肾体积呈负相关(r=-0.492,P=0.000)。肾叶间动脉RI与肾体积呈正相关(r=0.342,P=0.000)。结论研究组肾叶间动脉RI和肾弹性成像对预测1型糖尿病肾病有一定的诊断价值。Objective To investigate the predictive value of renal interlobar arte resistive index(RI)and renal parenchymal elastography for early type 1diabetic nephropathy.Methods A total of 90patients with type 1diabetes mellitus were selected as the study group,and 60healthy subjects as the control group.The renal interlobar artery resistive index and parenchymal elastography of right kidneys in both groups were measured,blood pressure,serum glucose,HbA1c,albuminuria,serum creatinine etc were collected.Data of the two groups were compared.The correlation between renal interlobar artery RI,renal parenchymal elastography and lab items above mentioned were analyzed.Results There were statistical inter-group differences in blood pressure,and biochemical parameters,except for serum creatinine(P<0.05).Kidney volume of the case group was bigger than that of the control group,and the renal cortext of the study group was thicker than that of the control group(P<0.001).The mean shear wave velocity of renal parenchyma was(2.91±0.32)m/s in the control group,and(2.50±0.50)m/s inC r:Othe study group.There was a statistically significant difference when the velocities were compared(P<0.001).As for the renal artery dynamics,there was a significant difference between the two groups when segmental artery velocities,segmental artery RI,and renal interlobar arte RI were compared(P<0.001).In the study group,the stiffness of renal parenchyma was negatively related with the kidney volume(r=-0.492,P<0.001),the renal interlobar artery RI was positively related with the kidney volume(r=0.342,P<0.001).Conclusions Renal interlobar artery RI and renal parenchymal stiffness have a certain value to predict type 1diabetic nephropathy.
