摘　　要：目的经阴道超声测量妊娠8~10^(+6)周孕妇平均子宫动脉搏动指数(meanUAPI),建立低危人群的meanUAPI正常参考范围,评价meanUAPI与不良妊娠结局的关系。方法本研究收集妊娠8~10^(+6)周孕妇1487例,经阴道超声测量meanUAPI。采用logistic回归分析meanUAPI与不良妊娠结局关系,建立低危人群meanUAPI正常参考范围;绘制受试者操作特征(ROC)曲线预测不良妊娠结局的效能。结果本研究中正常妊娠结局组1296例,不良妊娠结局组191例。logistic回归分析显示meanUAPI与妊娠结局相关(P<0.001),meanUAPI对先兆子痫(PE)有很高预测价值,得到8~8^(+6)周、9~9^(+6)周及10~10^(+6)周meanUAPI正常参考值范围分别为:1.5~3.1、1.3~2.9及1.2~2.8。结论本研究建立了妊娠8~10^(+6)周低危人群meanUAPI正常参考值,提示meanUAPI对PE发生有较高预测价值。Objective Mean uterine artery pulsatility index(meanUAPI)of pregnant women at 8-10^(+6) weeks gestation was measured by transvaginal ultrasonography,and the normal reference value of meanUAPI in low-risk population was established to evaluate the relationship between meanUAPI and adverse pregnancy outcomes.Methods MeanUAPI was measured by transvaginal ultrasonography in 1,487pregnant women at 8-10^(+6) weeks gestation.logistic regression was used to analyze the relationship between meanUAPI and adverse pregnancy outcomes,and the normal reference range of meanUAPI in low-risk population was established.ROC curve in predicting adverse pregnancy outcomes was drawn.Results In this study,there were 1296patients in the normal pregnancy outcome group and 191in the adverse pregnancy outcome group.logistic regression analysis showed meanUAPI was associated with pregnancy outcome(P<0.001).MeanUAPI had a high predictive value for pre-eclampsia(PE).The normal reference ranges of meanUAPI for 8-8^(+6) weeks,9-9^(+6) weeks and 10-10^(+6) weeks were 1.5-3.1,1.3-2.9and 1.2-2.8,respectively.Conclusions The normal reference value of meanUAPI in low-risk population at 8-10^(+6) weeks of gestation is established,suggesting that meanUAPI has a high predictive value for the occurrence of PE.
