超声评估女性盆腔静脉淤血与疼痛评分的相关性

摘　　要：目的分析女性盆腔静脉淤血(PVC)超声表现与临床疼痛评分的相关性。方法选取符合标准女性PVC患者89例。依据有无≥6个月的非周期性慢性盆腔痛症状分为症状组与无症状组,症状组42例,非症状组47例。采用Pearson相关性分析视觉模拟量表(VAS)与临床资料、盆腹腔血管超声结果,并行多元逐步回归分析。结果症状组左生殖静脉反流检出率明显高于非症状组(P<0.01),左肾静脉胡桃夹现象、左髂静脉压迫程度≥50%、下肢静脉曲张在两组间的检出率差异无统计学意义(P>0.05)。症状组盆腔静脉最大直径(DP)、左生殖静脉最大直径(DG)均显著大于无症状组(P<0.05),症状组左生殖静脉反流持续时间(TG)明显长于无症状组(P<0.01)。多元线性回归分析显示:TG、DG、DP与VAS评分依次呈显著独立正相关。12例症状组患者接受弹簧圈+硬化剂左卵巢静脉栓塞,术后1周VAS评分、DP较治疗前显著降低(P<0.001)。结论PVC患者TG、DG、DP与VAS疼痛评分呈明显正相关,可用于筛选适宜行介入栓塞的患者。Objective To analyze the correlation between ultrasound findings and clinical pain score in female pelvic venous congestion(PVC).Methods A total of 89female PVC patients meeting the criteria undergoing ultrasound examination were recruited.According to the the symptoms of aperiodic chronic pelvic pain symptoms≥6months,the patients were divided into symptomatic group and asymptomatic group,with 42cases in symptomatic group and 47 cases in non-symptomatic group.Pearson correlation analysis was used to analyze the correlation between VAS scores,clinical data and ultrasound results of pelvic and abdominal vessels,and multiple stepwise regression analyses analysis was further performed.Results The detection rate of reflux of left reproductive vein in symptomatic group was significantly higher than that in a-symptomatic group(P<0.01).There was no significant difference in the detection rates of nutcracker phenomenon of left renal vein,compression degree of left iliac vein≥50%and varicose veins of lower limbs between the two groups(P>0.05).The maximum diameter of pelvic vein(DP)and the maximum diameter of left reproductive vein(DG)in symptomatic group were significantly larger than those in asymptomatic group(P<0.05),and the duration of reflux of left reproductive vein in symptomatic group was significantly longer than that in asymptomatic group(P<0.01).Multiple linear regression analysis showed that TG,DG,DPand VAS scores had significant independent positive correlation with VAS scores.Twelve patients in symptom group were treated with left ovarian vein embolization with coil and sclerosing agent,and their VAS scores and DPwere significantly lower than those before treatment one week after operation(P<0.001).Conclusions TG,DG,and DPof PVC patients are significantly positively correlated with VAS pain scores,which can be used to screen patients suitable for interventional embolization.
