血清脂联素联合补体因子H对老年肾病综合征患者转归的预测价值评价

摘　　要：目的分析血清脂联素(APN)联合补体因子H(CFH)对老年肾病综合征患者转归的预测价值。方法选择本院自2020年1月—2022年6月收治的200例老年肾病综合征患者作为研究对象,根据治疗转归情况,分为转归组和未转归组。检测2组治疗前及治疗6个月的血清APN、CFH表达水平,分析治疗前血清APN、CFH与治疗6个月24 h尿蛋白定量的关系,通过受试者工作特征曲线(ROC)下面积(AUC)评价血清APN联合CFH对治疗未转归的预测效能。结果未转归组治疗前血清APN、CFH表达水平均低于转归组,差异均有统计学意义(P<0.05);未转归组治疗6个月血清APN、CFH表达水平均低于转归组,差异均有统计学意义(P<0.05);经Pearson相关性分析,老年肾病综合征患者治疗前血清APN、CFH与治疗6个月24 h尿蛋白定量均成负相关(P<0.05);经ROC曲线分析,治疗前血清APN联合CFH预测老年肾病综合征患者治疗未转归的AUC为0.920。结论血清APN联合CFH对老年肾病综合征患者转归的预测效能较好,值得临床予以重视应用。Objective This paper aims to analyze the predictive value of serum adiponectin(APN)combined with complement factor H(CFH)in elderly patients with nephrotic syndrome.Methods A total of 200 elderly patients with nephrotic syndrome admitted to our hospital from January 2020 to June 2022 were selected as the research objects.All patients were given specialized standard treatment for 6 months.According to the treatment outcome,they were divided into the outcome group and the non-outcome group.The serum levels of APN and CFH in the two groups were detected before treatment and 6 months after treatment.The relationship between serum APN and CFH before treatment and 24 h urinary protein quantification after 6 months of treatment was analyzed.The area under the receiver operating characteristic(ROC)curve(AUC)was used to evaluate the predictive efficacy of serum APN combined with CFH for non-treatment outcome.Results The serum levels of APN and CFH in the non-outcome group before treatment were lower than those in the outcome group,and the differences were statistically significant(P<0.05).The serum levels of APN and CFH in the non-outcome group were lower than those in the outcome group after 6 months of treatment,and the differences were statistically significant(P<0.05).Pearson correlation analysis showed that serum APN and CFH before treatment were negatively correlated with 24 h urinary protein quantification after 6 months of treatment in elderly patients with nephrotic syndrome(P<0.05).According to ROC curve analysis,the AUC of serum APN combined with CFH before treatment for predicting non-outcome in elderly patients with nephrotic syndrome was 0.920.Conclusion Serum APN combined with CFH has a good predictive effect on the outcome of elderly patients with nephrotic syndrome,which is worthy of clinical attention.
