摘 要:目的探讨介入栓塞术治疗对急性期前交通动脉瘤破裂患者脑血流灌注、氧化应激及神经功能的影响。方法将80例急性期前交通动脉瘤破裂患者随机分为开颅组和介入组各40例。开颅组行开颅夹闭术治疗,介入组行介入栓塞术治疗,比较两组患者的脑血流灌注情况[脑血流量(CBF)、脑血流容积(CBV)、脑血流平均通过时间(MTT)]、氧化应激反应[超氧化物歧化酶(SOD)、血清丙二醛(MDA)]及神经功能[美国国立卫生研究院卒中量表(NIHSS)评分]。结果术后1周,介入组MTT、MDA及NIHSS评分均低于开颅组,CBF、CBV、SOD高于开颅组(P<0.05)。结论与开颅夹闭术治疗相比,介入栓塞术治疗可有效改善急性期前交通动脉瘤破裂患者的脑血流灌注,缓解其氧化应激反应,减轻神经功能损伤。Objective To explore the effect of interventional embolization treatment on cerebral blood flow perfusion,oxidative stress and neurological function in patients with acute-stage anterior communicating aneurysm rupture.Methods 80 patients with acute-stage anterior communicating aneurysm rupture were randomly divided into craniotomy group and interventional group,with 40 cases in each group.The craniotomy group was treated with craniotomy clipping,and the intervention group was treated with interventional embolization.The cerebral blood flow perfusion[cerebral blood flow(CBF),cerebral blood volume(CBV),mean transit time(MTT)],oxidative stress response[superoxide dismutase(SOD),serum malondialdehyde(MDA)]and neurological function[National Institute of Health Stroke Scale(NIHSS)score]were compared between the two groups.Results 1 week after operation,the MTT,MDA and NIHSS score of the intervention group were lower than those of the craniotomy group,and the CBF,CBV and SOD were higher than those of the craniotomy group(P<0.05).Conclusions Compared with craniotomy clipping,interventional embolization can effectively improve cerebral blood flow perfusion,relieve the oxidative stress and reduce the neurological impairment in patients with acute-stage anterior communicating artery aneurysm rupture.
