摘 要:目的评价基于FA180全自动粪便寄生虫(卵)鉴定系统的粪便自动分析仪检测粪便中土源性线虫卵的效果。方法在已完成检测的2021年浙江省土源性线虫监测样本中选取452份进行方法学比较,其中虫卵阳性样本不少于20%,分别采用改良加藤厚涂片法和基于FA180全自动粪便寄生虫(卵)鉴定系统的粪便自动分析仪(全自动鉴定法)双盲检测土源性线虫虫卵,比较2种方法的检测时间和检测结果。结果全自动鉴定法平均检测时间为8.25 min·人/样本,改良加藤厚涂片法为11.26 min·人/样本,差异有统计学意义(t=25.60,P<0.001)。相较于改良加藤厚涂片,全自动鉴定法的Kappa值为0.86,阳性符合率为81.13%,阴性符合率为99.71%,总符合率为95.35%;当每克粪便虫卵数(EPG)≤48时,阳性符合率为44.44%,>720时阳性符合率为100.00%。结论基于FA180全自动粪便寄生虫(卵)鉴定系统的粪便自动分析仪检测用时短、人力成本低、与金标准符合率高,适用于人群土源性线虫病初筛;但在低虫卵密度下灵敏度不高,无法对虫卵进行定量分析。Objective This paper aims to evaluate the effectiveness of a fecal automatic analyzer based on the FA180 automatic fecal parasite(egg)identification system for detecting soil-transmitted nematode eggs in feces.Methods A total of 452 samples were collected from the tested Zhejiang Province soil-transmitted nematode surveillance fecal samples in 2021 for methodological comparison,of which more than 20%were positive for nematode eggs.The eggs of soil-derived nematodes were detected using the modified Kato Katz thick smear method and the automatic fecal analyzer based on FA180 automatic fecal parasite(egg)identification system,and the detection time and results of the two methods were compared.Results The average detection time of fecal automatic analyzer is 8.25 min·person/sample,and that of the modified Kato Katz thick smear was about 11.26 min·person/sample,and the differences were statistically significant(t=25.60,P<0.001).Compared with Kato Katz,the Kappa value of fecal automatic analyzer is 0.86,the positive coincidence rate is 81.13%,and the negative coincidence rate is 99.71%,the total coincidence rate is 95.35%;the coincidence rate is 44.44%when EPG is≤48,the coincidence rate is 100.00%when EPG is>720.Conclusion The fecal automatic analyzer based on the FA180 automatic fecal parasite(egg)identification system has the advantages of short detection time,low labor cost and high coincidence rate with the gold standard;however,there is insufficient sensitivity under low egg density and the eggs can’t be quantitatively analyzed.
