2019年—2022年文成县手足口病患儿病原谱及临床特征分析

摘　　要：目的分析2019年—2022年文成县手足口病患儿病原谱及临床特征。方法收集文成县2019年—2022年手足口病患儿数据,采集患儿粪便标本进行肠道病毒培养与分型检测,分析手足口病患儿肠道病毒病原分布情况及临床特征。结果2019年—2022年文成县累计报告手足口病例233例;手足口病发病时间集中于4月—6月;男性累计发病131例,女性102例;手足口病发病人群集中于1岁~3岁儿童,占总病例数的65.24%。标本培养显示,233份粪便标本中阳性标本130例,其中柯萨奇病毒A16型(Cox A16型)30例、肠道病毒71型(EV71)4例、其他肠道病毒96例。Cox A16型病毒引起的手足口病主要表现为发热、出疹、腹泻或便秘;EV71型和其他肠道病毒引起的手足口病主要表现为发热、出疹、食欲不振;Cox A16型、EV71型及其他肠道病毒引起的皮疹均位于口腔、四肢。结论2019年—2022年文成县其他肠道病毒引起的手足口病多于Cox A16型、EV71型,手足口发病时间集中于4月—6月,发病人群主要为1岁~3岁男性儿童,主要表现为发热、出疹,出疹部位多位于口腔及四肢。Objective This paper aims to analyze the pathogenic spectrum and clinical characteristics of children with hand-foot-mouth disease in Wencheng County from 2019 to 2022.Methods Data of hand-foot-mouth disease cases in Wencheng County from 2019 to 2022 were collected,and stool samples of children were collected for enterovirus culture and typing detection,and the distribution and clinical characteristics of enterovirus pathogens in children with hand-foot-mouth disease were analyzed.Results From 2019 to 2022,233 cases of hand-foot-mouth disease were reported in Wencheng County.The onset time of hand-foot-mouth disease was mainly from April to June.131 were male and 102 were female.The incidence of hand-foot-mouth disease was concentrated in children aged 1-3,accounting for 65.24% of the total cases.Specimen culture showed that 130 of 233 stool samples were positive,including 30 cases of coxsackievirus A16(Cox A16),4 cases of enterovirus 71(EV71)and 96 cases of other enteroviruses.The main manifestations of hand-foot-mouth disease caused by Cox A16 virus were fever,rash,diarrhea or constipation.The main manifestations of hand-foot-mouth disease caused by EV71 and other enteroviruses were fever,rash and anorexia.Rashes caused by Cox A16,EV71 and other enteroviruses were located in the mouth and limbs.Conclusion From 2019 to 2022,the number of cases of hand-foot-mouth disease caused by other enteroviruses in Wencheng County is more than that of Cox A16 and EV71.The onset time of hand-foot-mouth disease is mainly from April to June.The incidence population is mainly male children aged 1-3.The main manifestations are fever and rash.The eruption sites are mostly in mouth and limbs.
