外泌体miR-1246在绝经后骨质疏松症中的表达及临床意义

摘　　要：目的研究绝经后骨质疏松症(PMOP)患者血清外泌体中miR-1246的表达情况及其与患者临床资料之间的关系。方法收集2021年1月—2022年7月本院门诊与住院PMOP患者55例,同期45例健康绝经后妇女作为对照组,通过外泌体提取试剂盒分离PMOP组和对照组血清中的外泌体,采用蛋白免疫印迹检测外泌体标记的蛋白分子,透射电子显微镜鉴定血清外泌体形态;通过实时荧光定量PCR测量2组血清外泌体miR-1246的表达水平;并采用受试者工作特征(ROC)曲线评价血清外泌体miRNA-1246在PMOP中的诊断效能。结果2组血清来源的外泌体直径为40 nm~200 nm囊泡体,均表达蛋白CD9、CD63和CD81。患者血清外泌体miRNA-1246表达水平明显高于对照组(P<0.01)。外泌体miR-1246在PMOP患者中的表达与年龄、身体质量指数、骨代谢标志物、钙、镁磷、肌酐、甲状旁腺激素、白细胞介素-6和中性粒细胞计数都无关,然而与腰1至腰4椎体和股骨颈骨密度的T值成负相关关系(r=-0.36)。ROC曲线分析显示,血清外泌体miRNA-1246在PMOP中的诊断效能较大(AUC:0.79,P<0.01)。结论PMOP患者的血清外泌体miRNA-1246表达水平较高,对PMOP诊断及预后具有良好的参考价值。Objective This paper aims to study the expression of miR-1246 in serum exosomes of patients with postmenopausal osteoporosis(PMOP)and its relationship with patients'clinical data.Methods Fifty-five outpatients and inpatients with PMOP in our hospital from January 2021 to July 2022 were collected,and 45 healthy postmenopausal women were used as the control group during the same period.Exosomes in the serum of PMOP and the control group were separated by exosome extraction kits,and exosome-tagged protein molecules were detected by protein immunoblotting,and serum exosome morphology was identified by transmission electron microscopy.The expression of serum exosome miR-1246 was measured by real-time fluorescence quantitative PCR.The diagnostic efficacy of serum exosome miRNA-1246 in PMOP was evaluated by receiver operating characteristic(ROC)curve.Results The two groups of serum-derived exosomes were vesicles with a diameter of 40 nm-200 nm and expressed the proteins CD9,CD63 and CD81.The expression of serum exosome miRNA-1246 in patients was higher than that in control group(P<0.01).The expression of exosome miR-1246 in PMOP patients was not associated with age,body mass index,bone metabolic markers,calcium,magnesium,creatinine,parathyroid hormone,interleukin-6,and neutrophil counts,but was inversely correlated with T values of bone mineral density of lumbar 1 to lumbar 4 and femoral neck(r=-0.36).ROC curve analysis showed that serum exosome miRNA-1246 had high diagnostic efficacy in PMOP(AUC:0.79,P<0.01).Conclusion The expression of serum exosome miRNA-1246 in PMOP patients is high,which has good reference value for the diagnosis and prognosis of PMOP.
