比较2种血清学方法在胃癌及癌前状态中的筛查价值

摘　　要：目的比较胃蛋白酶原联合胃泌素-17(简称新“ABC法”)与新型胃癌筛查评分系统(简称新型评分法)在胃癌及癌前状态中的筛查价值。方法纳入本院2019年1月—2022年9月因胃部不适收治入院的患者932例,所有患者均做胃蛋白酶原、胃泌素-17、幽门螺杆菌检测,并进行胃镜和病理检查。根据血清学方法,将新“ABC法”和新型评分法均分为高危、中危、低危3组。结果2种血清学方法高、中危组胃癌检出率均高于低危组(P<0.05);新型评分法高、中危组癌前状态检出率均高于低危组(P<0.05)。新“ABC法”和新型评分法诊断胃癌的敏感度和特异度分别为72.97%、71.28%和89.19%、62.57%;新“ABC法”和新型评分法诊断癌前状态的敏感度和特异度分别为28.79%、71.36%和44.81%、70.44%。结论2种血清学方法均适用于胃癌筛查,其中新型评分法在筛查胃癌和癌前状态方面具有更高的价值。Objective This paper aims to compare the screening value of pepsinogen combined with gastrin-17(new“ABC method”)and the new gastric cancer screening scoring system(referred to as the new scoring method)in gastric cancer and precancerous state.Methods From January 2019 to September 2022,932 patients were admitted to the hospital due to stomach discomfort,and all patients were tested for pepsinogen,gastrin-17,Helicobacter pylori,and gastroscopy and pathological examination.According to the serological method,the new“ABC method”and the new scoring method were divided into three groups:high,medium and low risk.Results The detection rates of gastric cancer in the high-risk and intermediate-risk groups were higher than those in the low-risk group(P<0.05),and the precancerous detection rates in the high-risk and intermediate-risk groups were higher than those in the low-risk group(P<0.05).The sensitivity and specificity of the new“ABC method”and the new scoring method for the diagnosis of gastric cancer were 72.97%,71.28%,89.19% and 62.57%,respectively.The sensitivity and specificity of the new“ABC method”and the new scoring method for diagnosing precancerous state were 28.79%,71.36%,44.81% and 70.44%,respectively.Conclusion Both serological methods are suitable for gastric cancer screening,with the novel scoring method having higher value in screening for gastric cancer and precancerous states.
