摘 要:目的对羊水细胞载玻片原位培养过程进行风险评估,以探讨基于措施优先级的失效模式和效应分析(FMEA)的应用价值。方法应用基于措施优先级的FMEA,以过程为导向,风险评估羊水细胞载玻片原位培养的整个流程,对是否采取措施进行优先级排序,有针对性地开展风险控制。结果措施高优先级3项,措施中优先级3项,措施低优先级42项。对措施低优先级不额外采取措施,对措施中优先级加强控制,重点对措施高优先级制定进一步的预防和探测措施。改进后再评价结果显示,3项措施高优先级降至中优先级;3项措施中优先级评级未变。结论基于措施优先级的FMEA能够对羊水细胞载玻片原位培养过程进行有效地风险评估,并指导风险控制工作针对性地开展。本文可作为其他临床检验项目风险评估时的参考案例,尤其适用多步骤的手工项目风险管理。Objective This paper aims to evaluate the risk of the process of amniotic fluid cell culture in situ slides,in order to discuss the application value of FMEA based on action priority.Methods FMEA based on action priority was applied to assess the whole process of in situ culture of amniotic fluid cell slides,prioritize whether to take measures,and carry out targeted risk control.Results There are 3 high action priorities,3 medium action priorities,and 42 low action priorities.No additional measure was taken for the low action priority,control was strengthened over the medium action priority,and further preventive and detection measures were developed for high action priority.The results of re-evaluation after improvement show that 3 high action priorities were reduced to medium action priority,while the 3 medium action priorities were unchanged.Conclusion FMEA based on the priority of measures can effectively assess the risk of in situ culture process of amniotic fluid cell slides,and guide the risk control work.This paper can be used as a reference case for risk assessment of other clinical testing projects,especially for multi-step manual project risk management.
