摘 要:目的探讨男性抑郁症患者腰椎骨密度(bone mineral density,BMD)与正常人群的差异,分析患者BMD与激素(泌乳素、皮质醇)的相关性。方法回顾2021年7月至2022年2月,以抑郁症在首都医科大学附属北京安定医院住院的80例男性患者,年龄31~78岁,平均53.9岁。80例性别及年龄严格匹配的健康男性体检人群作为对照组。所有纳入的研究对象行腰椎定量CT(quantitative CT,QCT)BMD测量,并搜集血液泌乳素及皮质醇结果。采取独立样本t检验对比分析患病组和对照组之间BMD的统计学差异,进一步采用协方差分析校准两组间BMI的差异对结果的影响;χ^(2)检验对比男性抑郁症不同BMD组构成比与对照组的差异;采用Pearson相关分析评价抑郁症组BMD与泌乳素及皮质醇的相关性。结果男性抑郁症患者腰椎BMD明显低于健康对照组[(116.6±39.9)mg/cm^(3)vs.(135.2±37.2)mg/cm^(3),t=3.03,P<0.001];与对照组比较,男性抑郁症不同BMD组构成比差异有统计学意义(χ^(2)=10.255,P=0.005)。Pearson相关分析得出男性抑郁症患者腰椎BMD与泌乳素(r=0.036,P=0.777)及皮质醇(r=-0.199,P=0.114)均不具有相关性。结论与健康对照组相比,男性抑郁症患者BMD明显减低,与泌乳素、皮质醇无相关性。Objective To investigate the difference in lumbar bone mineral density(BMD)between male patients with major depressive disorder(MDD)and the normal population,and to analyze the correlation between BMD and hormones(prolactin and cortisol)in the patients.Methods Eighty male MDD patients who were hospitalized in Beijing Anding Hospital Affiliated to Capital Medical University from July 2021 to February 2022 were retrospectively selected as the disease group,with an average age of 53.9 years(range:31-78 years).80 healthy males were listed as the control group.All underwent quantitative computed tomography(QCT)measurement of BMD in the lumbar spine.Independent sample T-test was used to compare and analyze the statistical differences of basic data between the diseased group and the control group.The analysis of covariance was used to adjust the influence of BMI difference between the two groups.χ^(2)test was used to compare the difference of constituent ratio between the two groups.Pearson correlation analysis was used to evaluate the correlation between BMD and prolactin and cortisol in the MDD group.Results The lumbar spine BMD of male MDD patients was significantly lower than that of healthy controls[(116.6±39.9)mg/cm^(3)vs.(135.2±37.2)mg/cm^(3),t=3.03,P 0.001];there were significant differences in the constituent ratio between the two groups(χ^(2)=10.255,P=0.005).Pearson correlation analysis showed that there was no significant correlation between lumbar spine BMD and prolactin(r=0.036,P=0.777)and cortisol(r=-0.199,P=0.114)in male MDD patients.Conclusions Compared with healthy controls,male MDD patients have significantly lower BMD,but not associated with prolactin or cortisol.
