摘 要:目的 了解深圳地区不同分度慢性牙周炎(CP)患者缺氧诱导因子-1α(HIF-1α)、血管内皮生长因子(VEGF)、25-羟基维生素D[25-(OH)VitD]水平及HIF-1α基因(HIF-1α)G1790A位点多态性,并探讨其与牙周临床指标的相关性。方法 选择2020年3月至2022年5月CP确诊者137例(CP组),其中男性81例,年龄16~58岁,平均年龄31.89岁;女性56例,年龄17~56岁,平均年龄32.04岁;疾病程度,轻度79例,中度31例,重度27例。同期非CP健康体检人群115例(对照组),其中男性69例,年龄15~57岁,平均年龄30.28岁;女性46例,年龄16~59岁,平均年龄32.86岁。采用酶联免疫吸附分析法检测HIF-1α和VEGF水平,并运用C8000化学发光分析仪检测25-(OH)VitD水平,同时采用限制性片段长度多态性-聚合酶链式反应分析HIF-1α G1790A位点多态性。结果 CP组HIF-1α、VEGF水平(31.37 pg/mL±7.56 pg/mL,102.35 pg/mL±29.68 pg/mL)明显高于对照组(5.62 pg/mL±1.32 pg/mL,37.06 pg/mL±9.21 pg/mL),而25-(OH)VitD水平(23.54 mmol/L±7.82 mmol/L)明显低于对照组(48.09 mmol/L±5.76 mmol/L),差异有统计学意义(P <0.05)。HIF-1α、VEGF水平重度(79.49 pg/mL±7.58 pg/mL,203.95 pg/mL±26.34 pg/mL)>中度(32.50 pg/mL±3.17 pg/mL,103.64 pg/mL±20.78 pg/mL)>轻度(14.48 pg/mL±2.95 pg/mL,67.12 pg/mL±18.59 pg/mL),而25-(OH)VitD水平重度(7.65 mmol/L±4.02 mmol/L)<中度(18.09 mmol/L±8.35 mmol/L)<轻度(31.11 mmol/L±6.37 mmol/L),差异有显著统计学意义(P <0.001)。牙周袋深度(PD)、附着丧失(AL)、出血指数(BI)及菌斑指数(PLI)值重度>中度>轻度,差异有统计学意义(P <0.05)。经Pearson相关分析,CP组HIF-1α、VEGF水平与PD、BI、AL、PLI呈正相关,而25-(OH)VitD水平呈负相关(P <0.05)。CP组AA基因型和A等位基因频率(55.47%、65.69%)明显高于对照组(21.74%、31.30%),而GG基因型和G等位基因频率(24.09%、34.31%)明显低于对照组(59.13%、68.70%),差异有统计学意义(P <0.05),但GA基因型频率差异不明显,不同分度CP患者HIF-1α G1790A位Objective To investigate the levels of hypoxia-inducing factor 1α(HIF-1α),vascular endothelial growth factor(VEGF),25-hydroxyvitamin D[25-(OH)VitD]and polymorphism of HIF-1αgene(HIF-1α)G1790A in patients with different grades of chronic periodontitis(CP)in Shenzhen,and explore their correlation with periodontal clinical indicators.Methods From March 2020 to May 2022,a total of 137 patients diagnosed with CP(CP group)were enrolled,which included 81 males(aged 16-58 years old with mean age of 31.89 years old)and 56 females(aged 17-56 years old with mean age of 32.04years old).Among those patients,there were 79 mild cases,31 moderate cases and 27 severe cases.At the same time,115non-CP healthy subjects were set as control group,which included 69 males(aged 15-57 years old with mean age of 30.28years old)and 46 females(aged 16-59 years old with mean age of 32.86 years old).The levels of HIF-1αand VEGF were detected by enzyme-linked immunosorbent assay(ELISA),and level of 25-(OH)VitD was detected by C8000 chemiluminescence analyzer.The polymorphism of HIF-1αG1790A was analyzed by restriction fragment length polymorphism-polymerase chain reaction.Results The levels of HIF-1αand VEGF in CP group(31.37 pg/mL±7.56 pg/mL,102.35 pg/mL±29.68 pg/mL)were significantly higher than those in control group(5.62 pg/mL±1.32 pg/mL,37.06 pg/mL±9.21 pg/mL),while 25-(OH)VitD level(23.54 mmol/L±7.82 mmol/L)was significantly lower than that of control group(48.09 mmol/L±5.76 mmol/L),and the difference was statistically significant(P<0.05).HIF-1α,VEGF levels of severe cases(79.49 pg/mL±7.58 pg/mL,203.95 pg/mL±26.34 pg/mL)>that of moderate cases(32.50 pg/mL±3.17 pg/mL,103.64 pg/mL±20.78 pg/mL)>that of mild cases(14.48 pg/mL±2.95 pg/mL,67.12 pg/mL±18.59 pg/mL),and 25-(OH)VitD level was severe(7.65 mmol/L±4.02 mmol/L)<moderate(18.09 mmol/L±8.35 mmol/L)<mild(31.11 mmol/L±6.37 mmol/L),and the differences were statistically significant(P<0.001).The values of probing depth(PD),attachment loss(AL),bleed index(BI)and plague i
