摘 要:目的探讨C反应蛋白(mCRP)对hedgehog通路的影响及hedgehog通路对急性心肌梗死心功能的影响。方法通过结扎左前降支构建兔急性心肌梗死模型,将兔随机分为对照组、手术组(结扎左前降支)、手术+mCRP稳定组(结扎左前降支,通过使用CRP小分子抑制剂1,6双磷酸胆碱-己烷稳定mCRP)、手术+hedgehog激动组(结扎左前降支,通过使用hedgehog激动剂purmorphamine激动hedgehog通路),每组为10只,采用ELISA方法检测动物模型术后12 h血液中mCRP的表达情况,Western blot检测各组心肌hedgehog通路表达情况。术后8 w超声心动图检测其心功能,包括左室舒张末期内径(LVIDd)、射血分数(EF)、左室短轴缩短率(FS)、及心输出量(CO)。ELISA检测氨基末端B型利钠肽原(NT-proBNP)的含量。结果手术组及手术+hedgehog激动剂干预组的mCRP水平高于对照组及手术+mCRP稳定剂干预组mCRP水平,P<0.01。结扎左前降支并给予1,6双磷酸胆碱-己烷稳定mCRP后,hedgehog通路表达同对照组表达水平相当。结扎左前降支并给予hedgehog通路激动剂purmorphamine后,与其他手术组比较,NT-proBNP含量降低,兔LVIDd减小,EF、FS及CO增加,P<0.05。Pearson相关分析得出心肌梗死后hedgehog通路与mCRP表达水平呈正相关(R^(2)=0.94,P<0.05)。结论急性心肌梗死后,血液中mCRP及心肌组织中hedgehog蛋白表达水平均升高,且该蛋白表达与mCRP的表达呈正相关。这提示,mCRP介导hedgehog通路可能在急性心肌梗死发病过程中起着重要作用,激活hedgehog通路可能对心功能有保护作用。Objective To investigate the effect of C-reactive protein(mCRP)on hedgehog pathway and the effect of hedgehog pathway on cardiac function in rabbits with acute myocardial infarction.Methods The rabbit model of acute myocardial infarction was established by ligating the left anterior descending branch.The rabbits were randomly divided into a control group,a surgical group(ligating the left anterior descending branch),surgical+mCRP stable group(ligating the left anterior descending branch,stabilizing mCRP with the use of a small molecule inhibitor of CRP,choline diphosphate hexane),and surgical+hedgehog agonist group(ligating the left anterior descending branch,activating the hedgehog pathway with the hedgehog agonist purmorphamine).The number of rabbits in each group was 10,ELISA was used to detect the expression of mCRP in the blood of the animal model 12 h after operation,and Western blot was used to detect the expression of myocardial hedgehog pathway in each group.Echocardiography was performed 8 weeks after surgery to detect cardiac function,including left ventricular end diastolic diameter(LVIDd),ejection fraction(EF),left ventricular short axis shortening rate(FS),and cardiac output(CO).The content of N-terminal B-type natriuretic peptide(NT-proBNP)was detected by ELISA.Results The level of mCRP in surgical group and surgical+hedgehog agonist group was higher than that in control group and surgical+mCRP stable group(P<0.05).After ligating the left anterior descending branch,and stabilizing mCRP with the use of inhibitor of CRP,choline diphosphate hexane,the expression level of hedgehog pathway was similar to that of control group.After ligating the left anterior descending branch,and activating the hedgehog pathway with the hedgehog agonist purmorphamine,the expression level of NT-proBNP decreased,LVIDd decreased,EF,FS and CO increased compared with other surgical groups(P<0.05).According to Pearson correlation analysis,there was a positive correlation between hedgehog pathway and the expression of mCRP aft
