摘　　要：目的比较单侧双通道内镜技术(UBE)与经皮内镜术(PE)治疗神经根型颈椎病(CSR)的临床疗效。方法回顾性分析62例CSR患者,其中32例行UBE治疗(UBE组),30例行PE治疗(PE组)。比较两组患者一般资料、围术期资料、术前术后的VAS、NDI、JOA评分、责任间隙高度及稳定性。结果与PE组比较,UBE组镜下手术时间显著缩短(P<0.05),切口长度显著增加(P<0.05),两组住院时间比较,差异无统计学意义。随着术后时间推移,两组VAS评分和NDI评分均显著降低(P<0.05或<0.01),JOA评分均显著增高(P<0.01),但相应时间点两组间VAS、NDI及JOA评分比较,差异无统计学意义。两组术后责任间隙高度及稳定性与术前比较,差异无统计学意义。结论UBE与PE均能有效治疗CSR。与PE比较,UBE切口长度增加,但镜下手术时间明显缩短,是一种高效的手术方式。Objective To compare the clinical efficacy of unlateral biportal endoscopy(UBE)and percutaneous endoscopy(PE)for cervical spondylotic radiculopathy(CSR).Methods A retrospective analysis was performed on 38 cases of CSR.According to the treatment methods,32 patients were treated with UBE(UBE group)and 30 patients were treated with PE(PE group).The general data,perioperative data,preoperative and postoperative VAS,NDI,JOA scores,height and stability of responsibility gap were compared between the two groups.Results Compared with that in the PE group,the operation time under microscope was significantly shorter(P<0.05),and the incision length was significantly increased(P<0.05)in the UBE group.There was no significant difference in the length of hospital stay between the two groups.Over time,the VAS scores and NDI scores were significantly decreased(P<0.05 or<0.01),and JOA scores were significantly increased(P<0.01)in both groups,but there was no significant difference in VAS,NDI and JOA scores between the two groups at the corresponding time points.There was no significant difference in the height and stability of the responsibility gap between the two groups before and after the operation.Conclusion CSR can be effectively treated with both UBE and PE.Compared with PE,UBE has longer incision length,but significantly shorter operation time under the microscope,which is an efficient surgical method.
