摘 要:目的:回顾性分析住院男性2型糖尿病住院患者骨密度(BMD)情况,明确住院男性2型糖尿病患者骨质疏松患病率。方法:选取2012年6月—2020年1月在天津医科大学朱宪彝纪念医院住院的16790例男性2型糖尿病患者,均除外继发性骨质疏松。应用双能X线(GE LUNAR)测定患者腰椎L1~L4、全髋、股骨颈部位的骨密度。分析其BMD以及骨量异常的趋势。结果:住院男性2型糖尿病患者,峰值骨量出现在20~29岁;50岁以上骨量异常患病率显著增加(χ^(2)=133.926,P<0.05),其中股骨颈骨量异常患病率高于全髋(χ^(2)=201.411,P<0.05)与腰椎L1~L4(χ^(2)=743.993,P<0.05),而全髋骨量异常患病率高于腰椎L1~L4(χ^(2)=213.904,P<0.05)。结论:50岁以上住院男性2型糖尿病患者骨质疏松、骨量减少的发病率显著增高,尤其要重视髋部、股骨颈部位骨密度的检查。Objective:To investigate the bone mineral density(BMD)and the prevalence of osteoporosis in hospitalized male patients with type 2 diabetes.Methods:A total of 16790 male patients with type 2 diabetes who were hospitalized in Chu Hsien-I Memorial Hospital of Tianjin Medical University from June 2012 to January 2020 were selected,excluding secondary osteoporosis.Dual-energy X-ray(GE LUNAR)was used to determine bone mineral density of the patients′lumbar spine L1-L4,total hip,and femoral neck.The trend of BMD and abnormal bone mass was analyzed.Results:The peak BMD of hospitalized male patients with type 2 diabetes appeared at the age of 20 and 29 years old.The prevalence of abnormal bone mass in hospitalized male patients with type 2 diabetes over 50 years old increased significantly(χ^(2)=133.926,P<0.05),and the prevalence of abnormal bone mass in the femoral neck was higher than that in the total hip(χ^(2)=201.411,P<0.05)and lumbar spine L1-L4(χ^(2)=743.993,P<0.05),while the prevalence of abnormal bone mass in the total hip was higher than that in lumbar spine L1-L4(χ^(2)=213.904,P<0.05).Conclusion:The prevalence of osteoporosis and bone loss in hospitalized male patients with type 2 diabetes over 50 years old is significantly higher,especially the examination of BMD at the hip and femoral neck.
