摘　　要：目的对比研究阴茎彩色多普勒超声(PCDU)与5型磷酸二酯酶抑制剂(PDE5i)联合视听性刺激(AVSS)的阴茎硬度测量仪(RigiScan)检测结果,探讨PCDU在勃起功能障碍(ED)评估中的临床价值。方法回顾性分析2014年1月至2021年10月于中国中医科学院西苑医院男科门诊诊断为ED的1660例男性患者,所有患者均完成PCDU和PDE5i联合AVSS的RigiScan检测,对比其检测结果。结果根据PDE5i联合AVSS的RigiScan检测结果,将患者分为正常组(n=183)与异常组(n=1477),比较两组基线资料和各项PCDU检测指标,两组年龄、右侧收缩期峰值流速(PSV)、右侧舒张末期流速(EDV)、左侧PSV、左侧EDV比较,差异均具有统计学意义(P<0.05)。根据PDE5i联合AVSS的RigiScan检测的阴茎硬度分级将患者分为5组,其中0级457例,Ⅰ级421例,Ⅱ级369例,Ⅲ级391例,Ⅳ级22例。将该5组进行多组间比较,组间的年龄、右侧PSV、右侧EDV、右侧阻力指数(RI)、左侧PSV、左侧EDV、左侧RI比较,差异均具有统计学意义(P<0.05)。结论PCDU对区分心理性ED和血管性ED有重要意义,是诊断血管性ED的重要检查手段。Objective To explore the clinical value of penile color Doppler ultrasound(PCDU)in comparison with phosphodiesterase type-5 inhibitors(PDE5i)combined with the RigiScan-based audiovisual sexual stimulation(AVSS)test in evaluation of erectile function(ED).Methods A retrospective analysis was performed on 1,660 male patients diagnosed with ED in the Department of Andrology Outpatient of Xiyuan Hospital,China Academy of Chinese Medical Sciences from January 2014 to October 2021.All patients underwent PCDU and the PDE5i+RigiScan-based AVSS test.The results of the two tests were compared.Results By comparing the general characteristics and PCDU detection indexes of the normal and abnormal groups detected by the PDE5i+RigiScan-based AVSS test,statistical differences were found in age,right peak systolic velocity(PSV),right end-diastolic velocity(EDV),left PSV and left EDV of cavernous artery of the penis between the two groups(P0.05).According to the penile hardness grades obtained by the PDE5i+RigiScan-based AVSS test,the patients were divided into 5 groups,which 457 were in grade 0,421 were in gradeⅠ,369 were in gradeⅡ,391 were in gradeⅢ,and 22 were in gradeⅣ.Statistical differences were found in age,right PSV,right resistance index(RI),right EDV,left PSV,left EDV and left RI of cavernous artery of the penis among the groups(P0.05).Conclusions PCDU has great significance for distinguishing psychological ED from vascular ED and is an important examination method for the diagnosis of vascular ED.
