聚焦超声波结合体外冲击治疗特异性膝关节滑膜炎的临床疗效观察

摘　　要：目的探讨聚焦超声波结合体外冲击治疗特异性膝关节滑膜炎的临床价值。方法将2021年1月至2022年1月期间新乡市第一人民医院收治的92例特异性膝关节滑膜炎患者纳入本研究中,采用随机分组的方式,将患者分为观察组(n=46,采用聚焦超声波结合体外冲击治疗方案)与对照组(n=46,采用体外冲击治疗方案),两组患者在进行特异性膝关节滑膜炎对症治疗的同时均进行原发疾病的基础治疗。比较两组患者治疗前、治疗后1个月、2个月、3个月的疼痛视觉模拟(VAS)评分、膝关节AKSS评分,关节液炎症因子指标[超敏C反应蛋白(hs-CRP)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)]以及随访3个月后的膝关节滑膜炎复发率。结果治疗后,两组患者的VAS评分均随时间逐渐降低,且观察组患者各时间点的VAS评分均低于对照组,差异有统计学意义(P<0.05);AKSS评分均随时间而升高,且观察组患者治疗后各时间点的评分均高于对照组,差异有统计学意义(P<0.05)。观察组患者的关节液炎症因子指标情况均较对照组更低,差异有统计学意义(P<0.05),且随访3个月后,观察组患者的膝关节滑膜炎复发率更低,差异有统计学意义(P<0.05)。结论聚焦超声波结合体外冲击治疗特异性膝关节滑膜炎的疗效确切,可以缓解疼痛,改善膝关节功能,降低复发率。Objective To study the value of focused ultrasound combined with extracorporeal shock wave therapy for specific synovitis of knee joint.Methods A total of 92 patients with specific knee synovitis admitted to Xinxiang First People's Hospital from January 2021 to January 2022 were included in this study.Patients were randomly divided into observation group(n=46,using focused ultrasound combined with extracorporeal shock wave therapy)and control group(n=46,using extracorporeal shock wave therapy).Patients in both groups received symptomatic treatment for specific synovitis of the knee joint while receiving basic treatment for the primary disease.The pain visual analogue scale(VAS)score and knee joint AKSS score were compared between the two groups before and after treatment at 1,2 and 3 months.Articular fluid inflammatory factors[sensitive C-reactive protein(HS-CRP),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)]and recurrence rate of knee synovitis after 3 months of follow-up.Results After treatment,VAS score of 2 groups decreased gradually with time,and VAS score of observation group was lower than control group at each time point,with significant difference(P<0.05);AKSS score increased with time,and the score of observation group was higher than that of control group at each time point after treatment,with significant difference(P<0.05).The indexes of arthritic factors in the observation group were lower than those in the control group,with significant differences(P<0.05),and the recurrence rate of knee synovitis was lower in the observation group after 3 months of follow-up,with significant difference(P<0.05).Conclusion Focused ultrasound combined with extracorporeal shock wave therapy in the treatment of specific synovitis of knee joint is effective,can relieve pain,improve knee function,and reduce the recurrence rate.
