摘  要:目的分析血清糖类抗原125(CA125)、糖类抗原153(CA153)、神经元特异性烯醇化酶(NSE)联合检测对乳腺良恶性肿瘤的鉴别诊断价值及与肿瘤新生血管的相关性。方法选取2020年6月至2023年6月郑州市第三人民医院收治的80例乳腺肿瘤患者作为研究对象,首诊手术治疗经病理检查证实乳腺癌51例、乳腺结节29例,将其分为恶性组、良性组,并收集同期80例健康体检者作为对照组。比较三组及不同临床病理特征乳腺癌患者血清CA125、CA153、NSE水平,受试者工作特征曲线(ROC)分析血清CA125、CA153、NSE水平联合检测对乳腺良恶性肿瘤的鉴别诊断价值,分析血清CA125、CA153、NSE水平与淋巴结转移、临床分期、血管内皮生长因子(VEGF)的相关性。结果恶性组血清CA125、CA153、NSE水平>良性组>对照组,差异有统计学意义(P<0.05);ROC分析结果显示,血清CA125、CA153、NSE水平联合对乳腺良恶性肿瘤鉴别诊断的曲线下面积(AUC)为0.852,敏感度为80.39%、特异度为89.66%,高于各指标单独诊断,差异有统计学意义(P<0.05);相关性分析发现,乳腺癌患者血清CA125、CA153、NSE水平与淋巴结转移、临床分期、VEGF呈正相关,差异有统计学意义(P<0.05)。结论血清CA125、CA153、NSE水平与乳腺癌发生发展密切相关,联合检测有助于辅助临床鉴别诊断乳腺肿瘤良恶性及淋巴结转移、临床分期。Objective To analyze the value of serum carbohydrate antigen 125(CA125),serum carbohydrate antigen 153(CA153)and neuronal specific enolase(NSE)combined detection in differential diagnosis of benign and malignant breast tumors and its correlation with tumor neovascularization.Methods A total of 80 patients with breast cancer admitted to Zhengzhou Third People's hospital from June 2020 to June 2023 were selected as the study objects.After the first surgical treatment,51 cases of breast cancer and 29 cases of breast nodules confirmed by pathological examination were divided into malignant group and benign group,and 80 healthy patients of the same period were collected as the control group.Serum CA125,CA153 and NSE levels of breast cancer patients in the three groups and different clinicopathological characteristics were compared,and the value of combined detection of serum CA125,CA153 and NSE levels in the differential diagnosis of benign and malignant breast tumors was analyzed by receiver operating characteristic curve(ROC).The correlation of serum CA125,CA153,NSE levels with lymph node metastasis,clinical stage and vascular endothelial growth factor(VEGF)was analyzed.Results The serum levels of CA125,CA153 and NSE in malignant group were higher than those in benign group,while the serum levels of CA125,CA153,and NSE in the benign group were higher than those in the control group,and the difference was statistically significant(P<0.05).ROC analysis results showed that the area under the curve(AUC)of the combined serum CA125,CA153 and NSE levels in the differential diagnosis of benign and malignant breast tumors was 0.852,and the sensitivity and specificity were 80.39%and 89.66%,which were higher than that of each index alone,and the difference was statistically significant(P<0.05).Correlation analysis showed that serum CA125,CA153 and NSE levels in breast cancer patients were positively correlated with lymph node metastasis,clinical stage and VEGF,and the difference was statistically significant(P<0.05).Conclusion
