多层螺旋CT和HRCT诊断孤立性肺结节的价值分析

摘　　要：目的分析多层螺旋CT和高分辨率CT(HRCT)在孤立性肺结节诊断中的应用价值。方法选取济源职业技术学院附属医院2020年3月至2022年6月收治的92例孤立性肺结节患者作为研究对象,纳入研究对象的患者均接受多层螺旋CT、HRCT检查,将二者实际诊断结果与孤立性肺结节“金标准”检查病理活检进行对照,对其诊断结果及阳性征象检出率进行分析比较;另外对比HRCT、CT影像学特征。结果“金标准”病理活检诊断结果显示阳性例数为22例,阴性例数为70例,多层螺旋CT诊断结果显示阳性例数为34例、阴性例数为58例,HRCT诊断结果显示阳性例数为24例、阴性例数为68例,HRCT诊断准确度、灵敏度及特异度(97.83%、100.00%、97.14%)相较于多层螺旋CT(71.74%、68.18%、72.86%)较高,差异有统计学意义(P<0.05);HRCT、CT在分叶征、毛刺征、血管集束征检出方面对比,差异有统计学意义(P<0.05)。结论HRCT在孤立性肺结节的诊断效能高于多层螺旋CT,可给予后续治疗准确的指导,值得应用。Objective To analyze the application value of multi-slice spiral CT and high-resolution CT(HRCT)in the diagnosis of isolated pulmonary nodules.Methods A total of 92 patients with solitary pulmonary nodules admitted to the Affiliated Hospital of Jiyuan Vocational and Technical College from March 2020 to June 2022 were selected as the research objects.All patients included in the study underwent multi-layer spiral CT and HRCT examination,and the actual diagnosis results of the two were compared with the pathological biopsy of the"gold standard"examination of solitary pulmonary nodules.The diagnostic results and positive signs were analyzed and compared.In addition,the imaging features of HRCT and CT were compared.Results The"gold standard"pathological biopsy results showed 22 positive cases and 70 negative cases,multi-slice spiral CT diagnosis results showed 34 positive cases and 58 negative cases,and HRCT diagnosis results showed 24 positive cases and 68 negative cases.The diagnostic accuracy,sensitivity,and specificity of HRCT(97.83%,100.00%,97.14%)were higher than those of multi-slice spiral CT(71.74%,68.18%,72.86%),and the differences were statistically significant(P<0.05).There were statistically significant differences between HRCT and CT in the detection of lobulation signs,speculation signs,and vascular bundle signs(P<0.05).Conclusion HRCT is more effective than multi-slice spiral CT in the diagnosis of isolated pulmonary nodules,which can provide accurate guidance for subsequent treatment and is worthy of application.
