摘 要:目的收集2020—2021年间云南省的鼠伤寒沙门菌,了解其分子分型特征及耐药情况,为云南省鼠伤寒沙门菌防控提供参考。方法采用脉冲场凝胶电泳(pulsed-field gel electrophoresis,PFGE)方法对87株分离自云南省食源性疾病主动监测哨点医院感染患者的鼠伤寒沙门菌进行分型,应用BioNumerics7.6软件对酶切片段进行聚类分析,了解菌株的同源性特征;采用微量肉汤稀释法测定菌株对15种抗生素的药物敏感性。结果87株鼠伤寒沙门菌分子分型呈65种PFGE带型,相似度区间为68.1%~100.0%,有一种优势带型;对氨苄西林的耐药率最高,达79.3%,其次为氯霉素(65.5%)、甲氧苄啶/磺胺甲噁唑(63.2%);对头孢西丁最为敏感,敏感率为98.9%,其次是阿奇霉素(95.4%)、多黏菌素E(95.4%);多重耐药率为79.4%,优势耐药谱是CHL+TET+AMP+AMS+CFZ+SXT。结论云南省鼠伤寒沙门菌PFGE型别既表现出明显的遗传多样性,也存在明显的优势带型,同时多重耐药情况较为严重。PFGE分子分型特征结合长期的药敏监测数据,可以协助分析菌株的耐药趋势,对指导临床用药有重要意义。Objective To collect Salmonella typhimurium strains in Yunnan Province from 2020 to 2021 for understanding their molecular typing characteristics and drug resistance,and to provide references for prevention and control of Salmonella typhimurium in Yunnan Province.Methods Eighty-seven strains of Salmonella typhimurium isolated from patients in foodborne disease active surveillance sentinel hospitals in Yunnan Province were typed by pulsed-field gel electrophoresis.BioNumerics7.6 software was used to cluster and analyze the digested fragments so as to understand the homology characteristics of the strains.The drugsensitivities of the strains to 15 antibiotics were determined by micro-broth dilution method.Results Molecular typing of the 87 strains of Salmonella typhimurium showed 65 PFGE bands,with a similarity range of 68.1%-100%,and there was a dominant band type.The resistance rate to ampicillin was the highest,reaching 79.3%,followed by chloramphenicol(65.5%)and trimethoprim/sulfamethoxazole(63.2%).The strains were most sensitive to cefoxitin with a sensitivity rate of 98.9%,followed by azithromycin(95.4%)and polymyxin E(95.4%).The multidrug resistance rate was 79.4%,and the dominant resistance spectrum was CHL+TET+AMP+AMS+CFZ+SXT.Conclusion PFGE typing of Salmonella typhimurium in Yunnan Province showed obvious genetic diversity as well as obvious dominant band.Meanwhile,the multidrug resistance was serious.The molecular typing characteristics of PFGE combined with long-term drug susceptibility monitoring data can assist in analyzing the drug resistance trend of the strains,which is of great significance for guiding clinical drug use.
