自拟愈骨宁汤联合降钙素治疗创伤骨折后骨质疏松症的效果及对骨代谢指标的影响

摘　　要：目的:探讨自拟愈骨宁汤联合降钙素治疗创伤骨折后骨质疏松症的效果及对骨代谢指标的影响。方法:选取江西医学高等专科学校第一附属医院2021年1月—2022年6月收治的60例创伤骨折后骨质疏松症患者,依据随机数字表法分为对照组和治疗组,各30例。对照组给予降钙素治疗,治疗组联合自拟愈骨宁汤治疗。比较两组患者视觉模拟评分法(VAS)评分、临床疗效、骨密度(BMD)、骨代谢指标变化情况。结果:两组患者治疗前VAS评分、BMD及骨代谢指标比较,差异均无统计学意义(P>0.05);治疗2、3个月后,两组患者VAS评分均降低,且治疗组均低于对照组(P<0.05);治疗3个月后,两组L_(1~4)椎体、股骨颈的BMD均明显升高(P<0.05),且治疗组均较对照组改变更明显(P<0.05);治疗3个月后,治疗组患者Ⅰ型胶原氨基端延长肽(PⅠNP)、β-Ⅰ型胶原羧基末端交联肽(β-CTX)、骨钙素(BGP)、骨碱性磷酸酶(B-ALP)、抗酒石酸酸性磷酸酶-5b(TRACP-5b)水平均优于对照组(P<0.05);治疗组的临床疗效显著优于对照组(P<0.05)。结论:自拟愈骨宁汤联合降钙素治疗创伤骨折后骨质疏松症可抑制骨丧失,促进骨合成,显著改善骨代谢水平,提高BMD和临床疗效,改善了患者临床症状。Objective:To investigate the effect of self-designed Yuguning Decoction combined with Calcitonin in the treatment of osteoporosis after traumatic fracture and its influence on bone metabolism indexes.Method:Sixty patients with osteoporosis after traumatic fracture admitted to the First Affiliated Hospital of Jiangxi Medical College from January 2021 to June 2022 were selected and divided into control group and treatment group according to random number table method,with 30 patients in each group.The control group was combined with Calcitonin,and the treatment group was treated with self-designed Yuguning Decoction.Visual analogue scale(VAS)score,clinical efficacy,bone mineral density(BMD)and bone metabolism indexes were compared between the two groups.Result:There were no significant differences in VAS scores,BMD and bone metabolism indexes between the two groups before treatment(P>0.05);after 2 and 3 months of treatment,VAS scores of both groups were decreased,and the treatment group were combined than those of the control group(P<0.05);after 3 months of treatment,BMD of L_(1-4) vertebra and neck of femur were significantly increased in both groups(P<0.05),and the changes in the treatment group were more obvious than those in the control group(P<0.05);after 3 months of treatment,the levels of amino terminal prolongating peptide of typeⅠcollagen(PⅠNP),carboxyl terminal crosslinked peptide ofβ-Ⅰcollagen(β-CTX),osteocalcin(BGP),bone alkaline phosphatase(B-ALP)and tartrate-resistant acid phosphatase-5b(TRACP-5b)in treatment group were better than those in control group(P<0.05);the clinical efficacy of the treatment group was significantly better than that of the control group(P<0.05).Conclusion:Self-designed Yuguning Decoction combined with Calcitonin in the treatment of osteoporosis after traumatic fracture can inhibit bone loss,promote bone synthesis,significantly improve bone metabolism,increase BMD and clinical efficacy,and improve clinical symptoms of patients.
