摘  要:目的比较高压氧和常压氧处理对急性呼吸窘迫综合征(ARDS)大鼠肺损伤的保护作用及其机制。方法选取体重(200±20)g Wistar大鼠32只,按照随机数字表法分为4组,对照组(n=8)予以生理盐水注射,另外24只大鼠采用脂多糖(LPS)二次打击造模成功后分为模型组、常压氧组和高压氧组,每组8只。模型组建模成功后不进行其他处理,常压氧组给予2 h氧吸入,高压氧组予以0.25 MPa高压氧干预。分别于建模后和干预3 d后测定动脉血氧分压(PaO_(2))和吸入氧气浓度百分比(FiO_(2));检测血D-乳酸、内毒素和血浆二胺氧化酶(DAO)水平;测定支气管肺泡灌洗液(BALF)中白细胞介素(IL)-1β、IL-6、IL-10和肿瘤坏死因子-α(TNF-α)水平;检测大鼠肺组织中丙二醛(MDA)、过氧化氢酶(CAT)、谷胱甘肽(GSH)水平及超氧化物歧化酶(SOD)活性、髓过氧化物酶(MPO)活性;并进行大鼠肺组织病理评分,计算湿/干肺重量比(W/D)。结果高压氧组大鼠D-乳酸、内毒素、DAO水平、肺W/D比、肺组织病理评分、肺组织的MPO、MDA、GSH及灌洗液中IL-1β、IL-6和TNF-α水平明显低于模型组和常压氧组(P<0.05),PaO_(2)、PaO_(2)/FiO_(2)、CAT和SOD活性等指标明显高于模型组和常压氧组(P<0.05)。结论高压氧干预对ARDS大鼠的肺功能损伤具有保护作用,其机制可能与降低肺组织氧化应激水平、抑制炎症因子水平有关。Objective To compare the protective effects and mechanism of hyperbaric oxygen and atmospheric oxygen treatment on lung injury in rats with acute respiratory distress syndrome(ARDS).Methods A total of 32 Wistar rats weighing(200±20)g were selected and randomly divided into four groups according to the random number table method.The control group(n=8)was injected with normal saline,and the other 24 rats,after successfully modeled using LPS secondary blow,were divided into model group,atmospheric oxygen group,and hyperbaric oxygen group,with 8 rats in each group.The rats in the model group received no further treatment after successful modeling.The atmospheric oxygen group was given two hours of oxygen inhalation,while the hyperbaric oxygen group was given the intervention of 0.25 MPa hyperbaric oxygen.The arterial partial oxygen pressure(PaO_(2))and the percentage of inhaled oxygen concentration(FiO_(2))were measured by automatic blood gas analyzer after modeling and three days after intervention,respectively;the blood D-lactate concentration,endotoxin,and plasma diamine oxidase(DAO)activity were detected;the levels of interleukin(IL)-1,IL-1β,IL-6,IL-10,and tumor necrosis factor-α(TNF-α)in bronchoalveolar lavage fluid(BALF)were tested;the levels of lipid peroxidation in rat lung tissue were measured,including malondialdehyde(MDA),superoxide dismutase(SOD)activity,catalase(CAT),glutathione(GSH),and myeloperoxidase(MPO)activity;and pathological scoring of rat lung tissue was performed and the wet/dry lung weight ratio(W/D)was calculated.Results In the hyperbaric oxygen group,the levels of D-lactate,endotoxin,and DAO,lung W/D ratio,lung tissue pathological score,MPO,MDA,and GSH in lung tissue,and IL-1β,IL-6,and TNF-αin lavage fluid were all significantly lower than those of the model group and the atmospheric oxygen group(P<0.05),and the indicators,such as PaO_(2),PaO_(2)/FiO_(2),CAT,and SOD activity,were significantly higher than those of the model group and the atmospheric oxygen group(P<0.05).Conclusion Hype
