摘  要:目的观察产前MRI定量孕中晚期胎儿直窦(SS)横截面积的价值。方法回顾性分析228胎因产前超声疑诊中枢神经系统发育异常而接受颅脑MR检查的单胎妊娠中晚期胎儿,MRI显示194胎未见明显异常(正常组)、34胎颅后窝发育异常(异常组,含30胎单纯颅后窝池增宽及4胎脑静脉异常);测量胎儿SS横截面积,分析其与孕龄的相关性,根据正常组构建不同孕龄胎儿SS横截面积参考值,以Z-Score评估异常组胎儿SS横截面积偏离程度。结果孕中晚期正常胎儿SS横截面积与孕龄呈正相关(r_(s)=0.92,P<0.05),相应线性回归方程为:Y(SS横截面积)=-6.15+0.39X(孕龄)。异常组内30胎单纯颅后窝池增宽胎儿SS横截面积的|Z-Score|为1.46~1.91,均小于正常值2个标准差,与正常参考值无明显差异;4胎脑静脉异常胎儿SS横截面积的|Z-Score|为5.17~6.98,横截面积大于正常值5个标准差,与正常值差异较大。结论产前MRI可定量孕中晚期胎儿SS横截面积,有助于评估胎儿脑静脉系统发育。Objective To observe the value of prenatal MRI for quantitative evaluation on the cross-sectional area of fetal straight sinus(SS)in mid to late pregnancy.Methods Data of 228 single fetuses in mid to late pregnancy who underwent brain MR examination due to suspected abnormal development of central nervous system by prenatal ultrasound were retrospectively analyzed.Among them,MRI showed no obvious brain abnormalities in 194 fetuses(normal group),but abnormal development of posterior cranial fossa in 34 fetuses(abnormal group,including 30 with simple widening of posterior fossa pool and 4 with abnormal brain veins).Fetal SS cross-sectional area was measured based on prenatal MRI,and the correlation of SS with gestational age was analyzed.Then the reference values of fetal SS cross-sectional area of different gestational ages were constructed according to measurements of normal group,and Z-Score was used to evaluate the deviation of fetal SS cross-sectional area in abnormal group from normal fetuses.Results SS cross-sectional areas of normal fetuses in mid to late pregnancy were positively correlated with gestational ages(r_(s)=0.92,P<0.05),and the corresponding linear regression equation was as follows:Y(SS cross-sectional area)=-6.15+0.39X(gestational age).|Z-Score|of SS cross-sectional area in 30 fetuses with simple widening of posterior fossa pool were 1.46 to 1.91,all less than 2 standard deviations and not significantly different from the normal reference value.|Z-Score|of SS cross-sectional area in 4 cases with abnormal cerebral veins ranged from 5.17 to 6.98,and the cross-sectional areas were more than 5 standard deviations higher than normal value and were significantly different.Conclusion Prenatal MRI was valuable for quantification of fetal SS cross-sectional area in mid to late pregnancy,hence being helpful to evaluating development of fetal cerebral venous system.
