抑郁情绪在老年心力衰竭患者家庭支持与自我护理间的中介效应

摘　　要：目的探讨抑郁情绪在老年心力衰竭患者家庭支持和自我护理间的中介效应.方法选取201例在笔者所在医院心内科就诊的老年心力衰竭患者为研究对象,采用一般资料调查表、抑郁自评量表、家庭支持自评量表、心衰患者自我护理指数对其进行调查.结果老年心力衰竭患者抑郁得分为(46.92±12.85)分,家庭支持得分为(7.56±2.94)分,自我护理得分为(190.85±46.99)分.家庭支持与自我护理呈正相关(r=0.649,P<0.001),与抑郁情绪呈负相关(r=-0.513,P<0.001),抑郁情绪与自我护理呈负相关(r=-0.777,P<0.001).抑郁情绪在家庭支持与自我护理间具有中介效应,效应量占比47.22%.结论抑郁情绪在老年心衰患者家庭支持和自我护理间起到中介作用,医护人员应重视老年心衰患者抑郁情绪的筛查与干预,可通过提高家庭支持及减轻患者抑郁情绪来改善老年心衰患者自我护理现状.Objective To explore the mediating effect of depression between family support and self-care in elderly patients with heart failure.Methods Totally 201 elderly patients with heart failure and treated in the department of cardiology of the affiliated hospital of Jiangnan university were selected,and surveyed using a general information questionnaire,the self-rating depression scale,the perceived social support from family scale and the self-care of heart failure index.Results The depression,family support and self-care scores were 46.92±12.85,7.56±2.94 and 190.85±46.99,respectively.The family support score was positively correlated with the self-care score(r=0.649,P<0.001),but negatively correlated with the depression score(r=-0.513,P<0.001),while the depression score was negatively correlated with the self-care score(r=-0.777,P<0.001).Moreover,depression had a mediating effect between family support and self-care,accounting for 47.22%.Conclusion Depression plays an intermediary role between family support and self-care in elderly patients with heart failure.Medical staff should pay attention to screening and intervention of depression among such population,improve the self-care by relieving depression and improving family support.
