摘  要:目的探讨高低频超声检查对新生儿颅脑病变的诊断价值。方法抽取2019年10月至2021年9月濮阳市妇幼保健院收治的疑似颅脑病变的新生儿104例,均行高频超声和低频超声检查,以磁共振检查结果为"金标准",统计高频超声、低频超声及二者联合诊断的诊断结果和诊断效能,分析颅脑疾病检出率。结果104例疑似颅脑病变新生儿中,经磁共振检查确诊阳性63例,阴性41例;低频超声诊断出真阳性27例,真阴性25例;高频超声诊断出真阳性49例,真阴性31例;二者联合诊断出真阳性60例,真阴性39例。二者联合诊断灵敏度(95.24%,60/63)、特异度(95.12%,39/41)、准确度(95.19%,99/104)高于低频超声(42.86%,27/63;60.98%,25/41;50.00%,52/104)和高频超声(77.78%,49/63;75.61%,31/41;76.92%,80/104);漏诊率(4.76%,3/63)、误诊率(4.88%,2/41)低于低频超声(57.14%,36/63;39.02%,16/41)和高频超声(22.22%,14/63;24.39%,10/41),P<0.05。二者联合诊断的颅脑疾病总检出率(95.24%,60/63)、脑白质病变检出率(96.15%,25/26)高于低频超声(42.86%,27/63;34.62%,9/26)和高频超声(77.78%,49/63;61.54%,16/26),P<0.05。结论高低频超声联合检查诊断新生儿颅脑病变,能明显提升诊断灵敏度、特异度、准确度,减少漏诊和误诊情况的发生,可为临床制定针对性治疗方案提供参考依据。Objective To investigate the diagnostic value of high and low frequency ultrasound for neonatal brain diseases.Methods A total of 104 neonates with suspected brain diseases admitted to Puyang Maternal and Child Care Centers from October 2019 to September 2021 were selected.All children underwent high frequency and low frequency ultrasound for examination.Taking the results of magnetic resonance imaging as the gold standard,the diagnostic results and diagnostic efficacy of high frequency ultrasound,low frequency ultrasound and combination of them were counted,moreover,the detection rate of brain diseases was analyzed.Results Among the 104 neonates with suspected brain diseases,63 cases were positive,and 41 cases were negative confirmed by magnetic resonance imaging;27 cases were true positive,and 25 cases were true negative determined by low frequency ultrasound;49 cases were true positive,and 31 cases were true negative diagnosed by high frequency ultrasound;60 cases were true positive cases,and 39 cases were true negative diagnosed by the combination of the two methods.The sensitivity(95.24%,60/63),specificity(95.12%,39/41),and accuracy(95.19%,99/104)of the combined diagnosis of the two were higher than those of low frequency ultrasound(42.86%,27/63;60.98%,25/41;50.00%,52/104)and high frequency ultrasound(77.78%,49/63;75.61%,31/41;76.92%,80/104),P<0.05.The missed diagnosis rate(4.76%,3/63)and misdiagnosis rate(4.88%,2/41)of the combined diagnosis of the two were lower than those of low frequency ultrasound(57.14%,36/63;39.02%,16/41)and high frequency ultrasound(22.22%,14/63;24.39%,10/41),P<0.05.The total detection rate of brain diseases(95.24%,60/63)and the detection rate of white matter lesions(96.15%,25/26)diagnosed by the combined diagnosis of the two were higher than those diagnosed by low frequency ultrasound(42.86%,27/63;34.62%,9/26)and high frequency ultrasound(77.78%,49/63;61.54%,16/26),P<0.05.Conclusions The combined examination of high and low frequency ultrasound can significantly improve the sensitiv
