摘　　要：我国心血管疾病患病人数呈持续上升趋势,积极管理心血管危险因素如高血压、血脂异常、糖尿病以及超重/肥胖等,是遏制这种趋势的关键。多种心血管危险因素常共存于同一个体,对患者进行多重心血管危险因素综合达标管理十分重要。近20年来,大量随机对照研究结果提示,降压药物联合、降脂药物联合、降糖药物联合以及降压联合降脂、降糖联合降脂治疗,比单一危险因素管理能获得更多的心血管保护作用,由此,不同药理学成分组成的降压、降胆固醇、降糖的单片固定剂量复方制剂(fixed-dose combination,FDC)应运而生。此外,相关临床研究验证了在心血管疾病一级预防和二级预防中,与自由联合药物(free-equivalent combination,FEC)相比,FDC能显著提高患者服药依从性、增加长期服药持续性、提高多重心血管危险因素控制达标率,从而使患者的心血管预后改善更显著。基于循证证据,本专家共识提出,FDC可以成为我国更广泛人群进行心血管疾病预防的优化策略,推进临床医师在临床实践中规范处方FDC将助力实现“健康中国2030规划”的目标。The prevalence of cardiovascular disease in China is rising and the key for containing the trend is to manage the cardiovascular risk factors,such as hypertension,dyslipidemia,diabetes and overweight/obesity.Multiple cardiovascular risk factors often coexist in one patient and it is thus important to comprehensively manage these coexisting risk factors.In the past 20 years,numerous randomized controlled trials have demonstrated that the combination of antihypertensive drugs,lipidlowering drugs,and hypoglycemic drugs,as well as the combination of antihypertensive drugs and lipid-lowering drugs,hypoglycemic drugs and lipid-lowering drugs,can achieve more cardiovascular protective effects than the single risk factor management.Therefore,fixed-dose combination(FDC)pill,which is composed of different pharmacological components,such as antihypertensive,cholesterol lowering and glucose lowering components,has emerged.In addition,clinical studies have verified that FDC pill can significantly improve medication compliance,increase medication persistence and improve multiple cardiovascular risk factor control rate in the primary and secondary prevention of cardiovascular diseases compared with the free equivalent combination(FEC).Based on the evidence-based research,this expert consensus proposes that FDC pill can become an optimization strategy for cardiovascular disease prevention and treatment among wider population in China.Promoting the standardized application of FDC pill in clinical practice might help achieve the goal of"Healthy China 2030 Plan".
