摘  要:目的:采用双向两样本孟德尔随机化(MR)分析探讨心肌梗死与抑郁症的因果关系。方法:使用公开的全基因组关联研究汇总数据集,运用逆方差加权法(IVW)、加权中位数法、MR-Egger回归法、简单模式(simple mode法)和加权模式(weighted mode法)进行两样本MR分析,评估心肌梗死与抑郁症的因果关系。结果:MR分析结果表明,心肌梗死与抑郁症风险增加存在因果关系(IVW:OR=1.022,95%CI:1.008~1.037,P=0.002),抑郁症与心肌梗死风险增加不存在因果关系(IVW:OR=1.434,95%CI:0.931~2.209,P=0.102)。在心肌梗死对抑郁症的MR分析中,工具变量不存在水平多效性与异质性。在反向MR分析中,工具变量不存在水平多效性,MR-Egger回归分析发现工具变量之间存在微弱的异质性。使用留一法(leave-one-out法)进行的敏感性分析发现,MR分析结果稳健。结论:两样本MR分析结果显示,心肌梗死与抑郁症风险增加存在因果关系,但抑郁症与心肌梗死风险增加不存在因果关系。Objectives:To explore the causal relationship between myocardial infarction(MI)and depression by using bidirectional two-sample Mendelian randomization(MR)analysis.Methods:Using publicly available genome-wide association study(GWAS)summary datasets,MR was performed using inverse variance weighted(IVW),weighted median,MR-Egger regression,simple mode,and weighted mode methods to evaluate the causal relationship between MI and depression.Results:The MR results for a causal effect of MI on depression indicated a causal association between MI and an increased risk of depression(IVW:OR=1.022,95%CI:1.008-1.037,P=0.002).The MR results for a causal effect of depression on MI did not find a causal association between depression and an increased risk of MI(IVW:OR=1.434,95%CI:0.931-2.209,P=0.102).In the MR analysis for MI on depression,there was no horizontal pleiotropy and heterogeneity of instrument variables.In the reverse MR analysis,there was no horizontal pleiotropy of instrumental variables,and small heterogeneity between instrumental variables was found using MR-Egger regression.Sensitivity analysis was performed using the leave-one-out method,and the results of the MR analysis were robust.Conclusions:Results from our two-sample MR analysis indicate a causal association between MI and an increased risk of depression,but there is no causal association between depression and an increased risk of MI.
