摘 要:冠状动脉(冠脉)微循环是冠脉循环系统的重要组成部分,在冠脉疾病的发生和发展中起关键作用,相关病理生理研究依赖于微循环状态的准确评估。基于导丝的传统冠脉微循环阻力指数检测已有多年历史,但因操作复杂、耗时长、费用高等原因未能广泛用于临床。基于心血管影像、通过血液动力学模拟计算得到的新的微循环阻力指数,有望更为简便、高效地评估冠脉微循环的生理学状态,其准确度及临床应用价值尚待验证。本文就心血管影像衍生的冠脉微循环阻力指数的研究进展进行综述。Coronary microcirculation is an important component of the coronary circulatory system,which plays a key role in the occurrence and development of coronary artery diseases.Relevant pathophysiological studies rely on the accurate assessment of the microcirculation state.The traditional wire-based measurement of coronary microcirculation resistance index has been used for many years,but complicated procedures,longer assessment time and higher cost limited the widely use of this technique in daily clinical practice.A novel index of microcirculatory resistance derived from imaging based on hemodynamic simulation is expected to become a simpler and more efficient physiological index to assess coronary microcirculation.Its accuracy and clinical value remain to be verified.In this review,we summarized the research status and progress of the index of microcirculatory resistance derived from cardiovascular imaging.
