摘 要:目的 探讨系统性红斑狼疮(systemic lupus erythematosus, SLE)患者血清可溶性共刺激因子B7同源体-4(B7 nomolog 4,B7-H4)、外周血淋巴细胞表面诱导共刺激分子配体(inducible costimulator ligand, ICOSL)的表达水平与核因子KappaB(nuclear factor-κB,NF-κB)通路中IκB激酶(IκB-kinase, IKK)β间的关系。方法 将2020年4月至2021年12月50例SLE患者纳入研究组,选取同期来某院一般体检的50例健康人作为对照组,酶联免疫吸附法(enzyme linked immunosorbent assay, ELISA)检测血清B7-H4及诱导性共刺激分子配体(inducible costimulator ligand, ICOSL)表达水平,反转录聚合酶链反应(reverse tran-scription PCR,RT-PCR)法检测NF-κB通路中IKKβ mRNA表达水平。组间比较采用t检验,两两变量的相关性采用Pearson分析,绘制受试者操作特征(receiver operating characteristic, ROC)曲线分析联合B7-H4、ICOSL、IKKβ检测对SLE患者的诊断价值。结果 研究组血清可溶性B7-H4、ICOSL表达水平高于对照组,差异有统计学意义(t=-4.281、-31.439,P<0.05);研究组血清IKKβ mRNA表达水平高于对照组,差异有统计学意义(t=-5.256,P<0.05);Pearson相关系数显示B7-H4、ICOSL与IKKβ mRNA均存在显著正相关关系(r=0.782、0.766,P<0.05);ROC曲线显示联合检测诊断SLE曲线下面积为0.855,显著高于任意单一检测(Z=3.412,P<0.001),特异度为83.20%,敏感度为89.40%,有较高诊断效能。结论 SLE患者体内存在B7-H4、ICOSL和IKKβ mRNA高表达,3者呈正相关关系,实时检测B7-H4、ICOSL水平能准确评估疾病发展,对改善患者预后及抑制该病进展有重要意义。Objective To investigate the relationship between the expression level of soluble B7-H4 and ICOSL and IKKβkinase in NF-kB pathway in patients with systemic lupus erythematosus(SLE).Methods A total of 50 SLE patients from April 2020 to December 2021 were included in the study group,and 50 healthy subjects who received general physical examination in a given hospital during the same period were selected as the control group.The expressions of serum B7-H4 and ICOSL were detected by enzyme linked immunosorbent assay(ELISA).The mRNA expression of IKKβin NF-kB pathway was detected by reverse transcription PCR.Comparison between groups using t test,and Pearson was used to analyze the correlation between two variables.The diagnostic value of receiver operating characteristic curve(ROC)analysis combined with B7-H4,ICOSL and IKKβdetection for SLE patients was plotted.Results The expression levels of serum soluble B7-H4 and ICOSL in the study group were higher than those in the control group(t=-4.281,-31.439,P<0.05).The mRNA expression level of IKKβin the study group was higher than that in the control group,the difference had statistics significance(t=-5.256,P<0.05).Pearson correlation coefficient showed that B7-H4,ICOSL and IKKβmRNA were significantly positively correlated(r=0.782,0.766,P<0.05).ROC curve showed that the area under the curve of combined detection for the diagnosis of SLE was 0.855,which was significantly higher than that of any single detection(Z=3.412,P<0.001),with specificity of 83.20%and sensitivity of 89.40%,showing high diagnostic efficacy.Conclusion The high expression of B7-H4,ICOSL and IKKβmRNA in SLE patients is positively correlated with each other.Real-time detection of B7-H4 and ICOSL levels can accurately assess the progression of the disease,which is of great significance to improve the prognosis of patients and inhibit the further development of the disease.
