摘 要:目的研究3种血浆细胞因子白介素-6(interleukin 6,IL-6),白介素-2(interleukin 2,IL-2)和白介素-21(interleukin 21,IL-21)在人类免疫缺陷病毒1型(Human immunodeficiency virus type 1,HIV-1)感染者和正常人血浆表达水平及其在HIV-1感染病程进展中的相关性。方法收集2017年1月至2019年9月未经抗病毒治疗(anti-retroviral therapy,ART)的HIV-1感染者27例(HIV组);ART后血浆病毒载量低于检测限,CD4>350cells/μl者22例(HIVⅠ组);ART后血浆病毒载量>1000copies/ml,CD4<350cells/μl者31例(HIVⅡ组),另选30例健康体检者为对照组。采用酶联免疫吸附试验(enzyme linked immunosorbent assay,ELISA)检测3种细胞因子水平,流式细胞术检测CD4细胞数量,实时聚合酶链法测定血浆病毒载量。结果HIV-1感染者血浆IL-6水平显著高于对照组,IL-2和IL-21水平显著低于对照组。相关性分析显示,IL-6水平与CD4细胞计数呈负相关,与病毒载量呈正相关;IL-2和IL-21水平与CD4细胞计数呈正相关,与病毒载量呈负相关。结论HIV-1感染者血浆细胞因子IL-6、IL-2和IL-21出现异常变化;HIV-1感染者血浆IL-6变化最为活跃和显著,提示IL-6水平变化可能与疾病的进展密切相关。Objective To investigate the plasma expression levels of three plasma interleukin 6(IL-6),interleukin 2(IL-2),and interleukin 21(IL-21)in Human immunodeficiency virus type 1(HIV-1)infected and normal individuals and their correlation in the progression of HIV-1 infection.Methods From January 2017 to September 2019,a total of 27 HIV-1 infected patients without anti-retroviral therapy(ART)were collected as HIV group,22 with a viral load below the detection limit and CD4>350cells/μl after ART were collected as HIVⅠgroup,31 with plasma viral load>1000copies/ml and CD4 cell count<350cells/μl after ART as HIVⅡgroup,and 30 healthy subjects were selected as control group.Enzyme-linked immunosorbent assay(ELISA)was used to detect the levels of three cytokines.Flow cytometry was used to detect the number of CD4 cells,and the level of plasma HIV RNA was determined by real-time polymerase chain reaction.Results The plasma IL-6 level was significantly higher and the levels of IL-2 and IL-21 were significantly lower in HIV-1 infected than in the control group.Correlation analysis showed that IL-6 level was negatively correlated with CD4 cell count and positively correlated with plasma viral load.The levels of IL-2 and IL-21 were positively correlated with CD4 cell count and negatively correlated with plasma viral load.Conclusion The abnormal changes in plasma cytokines levels of IL-6,IL-2,and IL-21 are observed in HIV-1 infected individuals.The changes of IL-6 in HIV-1 infected are most active and significant,suggesting that the changes of IL-6 may be closely related to the progression of the disease.
