赋权视角下链式管理模式在川崎病患儿居家护理中的应用

摘　　要：目的:探讨赋权视角下链式管理模式在川崎病(KD)患儿居家护理中的应用效果。方法:选取2019年2月1日~2021年3月31日收治的77例KD患儿,采用随机信封法分为对照组38例和观察组39例,对照组采用常规延续护理,观察组采用赋权视角下链式管理模式;比较两组干预前后家庭负担[采用家庭负担会谈量表(FBIS)]、监护人对疾病的认知水平、家庭功能[采用家庭功能评定量表(APGAR)]及并发症发生情况。结果:干预后,两组FBIS各维度评分均低于干预前(P<0.05),且观察组低于对照组(P<0.01);干预后,两组监护人KD疾病知识调查问卷各维度评分均高于干预前(P<0.05),且观察组高于对照组(P<0.01,P<0.05);干预后,两组APGAR各维度评分均高于干预前(P<0.05),且观察组高于对照组(P<0.01);观察组患儿杨梅舌、颈部淋巴结肿大、高热发生率均低于对照组(P<0.05)。结论:将赋权视角下链式管理模式应用于KD患儿居家护理中,可减轻患儿家庭负担,提高监护人对疾病的认知程度,同时还可降低患儿并发症的发生率,提高患儿家庭功能水平。Objective:To explore the application effect of chain management model under empowerment perspective in the home care of children with Kawasaki disease(KD).Methods:A total of 77 children with KD admitted were selected between February 1,2019 and March 31,2021 and were divided into the control group with 38 cases and the control group with 39 cases by the random envelope method.The control group adopted conventional continuous nursing and the observation group adopted chain management model under empowerment perspective for intervention.The family burden by Family Burden Interview Scale(FBIS),cognition level of the children′s guardians toward disease,family function by APGR,and complications were compared between the two groups before and after intervention.Results:After intervention,the FBIS dimensional scores in both groups were lower than those before intervention(P<0.05),and those in the observation group were lower than those in the control group(P<0.01).After intervention,the scores of various dimensions of KD disease knowledge questionnaire in both groups of children′s guardians were higher than those before intervention(P<0.05),and those in the observation group were higher than those in the control group(P<0.01,P<0.05).After intervention,the scores of various dimensions of APGAR in both groups were higher than those before intervention(P<0.05),and those in the observation group were higher than those in the control group(P<0.01).After intervention,the scores of bayberry tongue neck lymphadenopathy and high fever incidence in the observation group were lower than those in the control group(P<0.05).Conclusion:The application of chain management model under empowerment perspective in the home care of children with KD can reduce the family burden,increase the cognition degree of children′s guardians toward disease,and reduce the incidence of complications and improve the family function level of children.
