4R危机管理理念在肺结核患者中的应用

摘　　要：目的:探讨4R危机管理理念在肺结核患者中的应用效果。方法:将2019年12月1日~2020年12月31日收治的55例肺结核患者作为对照组,实施常规护理干预;将2021年1月1日~12月31日收治的55例肺结核患者作为观察组,在对照组基础上实施4R危机管理理念干预,于入院当日、出院前一天比较两组肺功能指标[包括肺活量(VC)、潮气容积(TV)、最大通气量(MVV)、功能残气量(FRC)]、自我管理能力[采用自我护理能力测定量表(ESCA)]及情绪变化[采用正性负性情绪量表(PA-NAS)],并发症发生情况。结果:出院前一天,两组肺功能指标、PANAS评分及ESCA评分均优于入院当日(P<0.05,P<0.01),且观察组优于对照组(P<0.05,P<0.01);观察组并发症发生率低于对照组(P<0.05)。结论:实施4R危机管理理念干预能够改善肺结核患者的肺功能指标,提高自我管理能力,改善负性情绪,降低其并发症发生率。Objective:To explore the application effect of 4R crisis management concept in the tuberculosis patients.Methods:During December 1,2019 to December 31,2020,55 cases of tuberculosis patients were as the control group,routine nursing intervention was given;During January 1,2021 to December 31,2021,55 cases of tuberculosis patients were as the observation group,on the basis of the control group,4R crisis management intervention was given on the admission day,1 d before discharge,comparison was made between the two groups on lung function indexes including forced vital capacity(VC),tidal volume(TV),maximum ventilation volume(MVV),the functional residual capacity(FRC),self-management ability by Exercise of Self-care Agency(ESCA)and mental changes by the Positive and Negative Affect Scale(PANAS),and complications.Results:One day before discharge,the lung function indexes,PANAS scores and ESCA scores were better in both groups than those on the day of admission(P<0.05,P<0.01),and those in the observation group were better than those in the control group(P<0.05,P<0.01).The incidence of complication was lower in the observation group than that in the control group(P<0.05).Conclusion:The implementation of the 4R crisis management intervention can improve the lung function indexes of the tuberculosis patients,improve the ability of self-management,improve the negative mood,and reduce the incidence of complications.
