低能量体外冲击波联合膀胱药物灌注治疗IC/BPS的临床观察

摘　　要：目的探讨低能量体外冲击波(Li-ESWT)联合膀胱药物灌注治疗间质性膀胱炎/膀胱疼痛综合征(IC/BPS)的临床疗效与安全性。方法选取2018年2月至2020年10月来本院就诊的36例女性IC/BPS患者,随机分成对照组与治疗组,每组各18例。对照组采用膀胱药物灌注治疗,治疗组采用膀胱药物灌注联合Li-ESWT治疗。所有患者治疗前后均行O'Leary-Sant间质性膀胱炎症状指数(ICSI)和问题指数评分(ICPI),并采用泌尿生殖系统疼痛评分(GUPI)和视觉模拟评分法(VAS)评估疗效。结果经治疗后,治疗组患者的O'Leary-Sant症状评分(OSS)、ICSI、ICPI、GUPI及VAS评分均优于对照组(均P<0.001),且两组患者治疗后的各项评分均优于治疗前(均P<0.001);治疗组患者的临床改善有效率明显高于对照组(83.33%vs.44.44%,P=0.034)。结论Li-ESWT联合膀胱药物灌注治疗IC/BPS安全有效,为临床治疗IC/BPS提供了一种有效的治疗方法。Objective To investigate the efficacy and safety of Low Intensity extracorporeal shock wave therapy(Li-ESWT)combined with intravesical drug therapy for interstitial cystitis/bladder pain syndrome(IC/BPS).Methods A total of 36 female IC/BPS patients who came to our hospital from February 2018 to October 2020 were randomly divided into control group and treatment group.The control group was treated with bladder drug infusion,and the treatment group was treated with bladder drug infusion combined with Li-ESWT.O'Leary-Sant questionnaire interstitial cystitis symptom index(ICSI)and problem index score(ICPI),genitourinary pain index(GUPI),and visual analogue scale(VAS)were observed and recorded for IC/BPS symptom assessment before and after treatments.Results After treatment,the scores of O'Leary-Sant characteristic scroes(OSS),ICSI,ICPI,GUPI and VAS in the treatment group were better than those in the control group(all P<0.001),and the scores after treatment were better than those before treatment(all P<0.001).The clinical improvement rate of the treatment group was significantly higher than that of the control group(83.33%vs.44.44%,P=0.034).Conclusions Li-ESWT combined with bladder drug perfusion is safe and effective in the treatment of IC/BPS,which provides an effective treatment method for the clinical treatment of IC/BPS.
