全体积CT纹理分析预测ESWL治疗输尿管结石疗效的临床应用价值

摘　　要：目的探讨全体积CT纹理分析(CTTA)预测体外冲击波碎石术(ESWL)治疗输尿管结石疗效的临床应用价值。方法回顾性分析2018年6月至2022年6月在无锡市人民医院门诊行ESWL治疗的86例输尿管结石患者的临床资料,ESWL术后2周复查,未见结石残留或者残留结石长径<3mm者纳人成功组(53例),否则纳人失败组(33例)。提取整个结石全体积的每一个像素CT值,计算出全体积CT纹理参数。分析全体积CT纹理参数和ESWL疗效的相关性。结果ESWL的治疗成功率为61.63%。结石HU值、结石体积与ESWL的疗效显著相关(均P<0.05)。成功组的全体积CT纹理参数中75百分位数和熵、均值、中值、全距、极大值、和、25百分位数、75百分位数均低于失败组(均P<0.05)。结论全体积CT纹理参数中,熵、均值、中值、全距、极大值、和、25百分位数、75百分位数有望被用来预测ESWL的疗效。Objective To discuss the clinical application in predicting the efficacy of extracorporeal shock wave lithotripsy(ESWL)of ureteral stone.Methods The data of 86 patients with ureteral stones who were received ESWL in the outpatient department of Wuxi People's Hospital from June 2018 to June 2022 were retrospectively collected.Two weeks after ESWL,no residual stones or residual stones<3 mm were included in the success group(53 cases),otherwise they were included in the failure group(33 cases).The CT value of each pixel in the whole stone volume was extracted and the whole volume CT texture parameters were calculated.The correlation between the whole volume CT texture parameters and the efficacy of ESWL was analyzed.Results The success rate of ESWL was 61.63%.Stone HU value,stone volume were significantly associated with ESWL success(all P<0.05).Among the whole volume CT texture parameters,Entropy,mean,median,full range,maximum,sum,25th percentile and 75th percentile in the success group were lower than those in the failure group,and the differences were statistically significant(all P<0.05).Conclusions Among the whole volume CT texture parameters,Entropy,mean,median,full range,maximum,sum,25th percentile and 75th percentile are expected to be selected to help predicting the efficacy of ESWL.
