摘 要:目的探讨乙型肝炎病毒(HBV)感染患者HBV-DNA载量与外周血CD4+CD25+、白细胞介素33(IL-33)水平、肝组织病理的相关性。方法选取成都中医药大学附属医院自2021年1月至2022年12月收治的104例慢性乙型肝炎(CHB)患者为研究对象。依据HBV-DNA载量将患者分为高载量组(HBV-DNA载量>1.0×10^(5) IU/ml,n=71)与低载量组(1.0×10^(3) IU/ml<HBV-DNA载量≤1.0×10^(5) IU/ml,n=33)。比较两组患者外周血CD4+CD25+、IL-33水平与肝组织病理。采用Spearman法检验CD4+CD25+、IL-33与HBV-DNA的相关性。采用多因素Logistic回归模型分析检测HBV-DNA载量的影响因素。结果高载量组患者CD4+CD25+、IL-33均高于低载量组,差异均有统计学意义(P<0.05)。两组患者纤维化分期、肝炎症活动分级比较,差异均有统计学意义(P<0.05)。Spearman相关性分析显示,CD4+CD25+、IL-33与HBV-DNA载量呈正相关性(r=0.492、0.513,P<0.05)。多因素Logistic回归分析显示,CD4+CD25+、IL-33、纤维化分期、炎症分级均为HBV-DNA载量的影响因素(P<0.05)。结论HBV感染患者HBV-DNA载量与外周血CD4+CD25+、IL-33水平、肝组织病理密切相关。Objective To investigate the correlation between HBV-DNA load and peripheral blood CD4+CD25+,interleukin-33(IL-33)levels and liver histopathology in patients infected with hepatitis B virus(HBV).Methods A total of 104 patients with chro-nic hepatitis B(CHB)admitted to the Affiliated Hospital of Chengdu University of Traditional Chinese Medicine from January 2021 to December 2022 were selected as the study objects.According to HBV-DNA load,patients were divided into high load group(HBV-DNA load>1.0×10^(5) IU/ml,n=71)and low load group(1.0×10^(3) IU/ml<HBV-DNA load≤1.0×10^(5) IU/ml,n=33).The levels of CD4+CD25+and IL-33 in peripheral blood and liver histopathology were compared between the two groups.Spearman method was used to determine the correlation between CD4+CD25+,IL-33 and HBV-DNA.Multivariate Logistic regression model was used to an-alyze the factors affecting the detection of HBV-DNA load.Results CD4+CD25+and IL-33 in high load group were higher than those in low load group,the differences were statistically significant(P<0.05).There were significant differences in fibrosis stage and hepatitis activity grade between two groups(P<0.05).Spearman correlation analysis showed that CD4+CD25+and IL-33 were posi-tively correlated with HBV-DNA load(r=0.492,0.513,P<0.05).Multivariate Logistic regression analysis showed that CD4+CD25+,IL-33,fibrosis stage and inflammation grade were all influencing factors of HBV-DNA load(P<0.05).Conclusion HBV-DNA load in patients with HBV infection is closely related to the levels of CD4+CD25+,IL-33 and liver histopathology in peripheral blood.
