解毒化痞膏联合疏肝降脂片治疗非酒精性脂肪性肝病伴胁痛回顾性研究

摘　　要：目的:观察解毒化痞膏联合疏肝降脂片治疗非酒精性脂肪性肝病(NAFLD)的临床疗效。方法:收集2018年7月-2019年7月在广州中医药大学第二附属医院(广东省中医院)治疗的95例NAFLD伴胁痛患者的病历资料,根据是否采用解毒化痞膏治疗分为解毒化痞膏组63例和常规治疗组32例。2组均给予西药治疗及常规护理,常规治疗组在此基础上给予疏肝降脂片治疗,解毒化痞膏组在常规治疗组基础上加用解毒化痞膏治疗,2组均治疗12周。比较2组治疗前后胁痛程度[视觉模拟评分法(VAS)评分]、血脂指标[总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)]、脂肪肝严重程度[受控衰减参数(CAP)]、肝功能指标[谷草转氨酶(AST)、谷丙转氨酶(ALT)]及体质量指数(BMI)。结果:治疗后,2组VAS评分均较治疗前降低(P<0.05),解毒化痞膏组VAS评分低于常规治疗组(P<0.05)。2组CAP值及TC、TG、LDL-C水平均较治疗前降低(P<0.05),HDL-C水平均较治疗前升高(P<0.05);解毒化痞膏组CAP值及TC、TG、LDL-C水平均低于常规治疗组(P<0.05),HDL-C水平高于常规治疗组(P<0.05)。2组血清ALT、AST水平及BMI均较治疗前降低(P<0.05),解毒化痞膏组血清ALT、AST水平及BMI均低于常规治疗组(P<0.05)。结论:解毒化痞膏联合疏肝降脂片治疗能有效缓解NAFLD患者的胁痛症状,降低血脂水平,效果优于单纯使用疏肝降脂片治疗。Objective:To observe the clinical effect of Jiedu Huapi Ointment combined with Shugan Jiangzhi Tablets for nonalcoholic fatty liver disease(NAFLD).Methods:The medical records of 96 patients with NAFLD complicated with hypochondriac pain who were treated in The second Affiliated Hospital of Guangzhou University of Chinese Medicine(Guangdong Provincial Hospital of Chinese Medicine)from July 2018 to July 2019 were collected.The patients were divided into the Jiedu Huapi Ointment group and the routine treatment group according to whether they were treated with Jiedu Huapi Ointment,with 63 and 32 cases in each group respectively.Both groups were treated with western medicine and routine nursing;the routine treatment group was additionally treated with Shugan Jiangzhi Tablets,and the Jiedu Huapi Ointment group was additionally treated with Jiedu Huapi Ointment based on the treatment of the routine treatment group.Both groups were treated for 12 weeks.The degree of hypochondriac pain[Visual Analogue Scale(VAS)scores],blood lipid indexes[total cholesterol(TC),triglyceride(TG),low-density lipoprotein cholesterol(LDL-C),and high-density lipoprotein cholesterol(HDL-C)],severity of fatty liver[controlled attenuation parameter(CAP)],liver function indexes[aspartate aminotransferase(AST)and alanine aminotransferase(ALT)],and body mass index(BMI)were compared between the two groups before and after treatment.Results:After treatment,VAS scores in the two groups were decreased when compared with those before treatment(P<0.05),and VAS score in the Jiedu Huapi Ointment group was lower than that in the routine treatment group(P<0.05).The CAP values and levels of TC,TG,and LDL-C in the two groups were decreased when compared with those before treatment(P<0.05),and HDL-C levels were increased when compared with those before treatment(P<0.05);CAP value and levels of TC,TG,and LDL-C in the Jiedu Huapi Ointment group were lower than those in the routine treatment group(P<0.05),and HDL-C level was higher than that in the routine treatment
