三维调颈手法联合针刀治疗神经根型颈椎病红外热成像临床研究

摘　　要：目的:观察三维调颈手法联合针刀治疗神经根型颈椎病的临床疗效及其对患者病变区域红外热成像的影响。方法:选取128例符合纳入标准的神经根型颈椎病患者,根据随机数字表法分为4组各32例。A组为对照组、B组为三维调颈手法组、C组为针刀组、D组为三维调颈手法联合针刀组。4组均治疗3周,比较4组治疗前后颈椎功能障碍指数(NDI)及红外热成像改变情况。结果:治疗后及治疗后3个月,B、C、D组NDI评分分别与治疗前相比较,差异均有统计学意义(P<0.05);治疗后及治疗后3个月,B、C、D组颈部温差分别与治疗前比较,差异均有统计学意义(P<0.05);且D组颈部温差减小较B、C组明显,差异均有统计学意义(P<0.05);A组治疗后及治疗后3个月颈部温差分别与治疗前比较,差异均无统计学意义(P>0.05)。结论:三维调颈手法联合针刀治疗能改善颈椎功能,临床疗效优于单纯三维调颈手法或针刀治疗。Objective:To observe the clinical effect of the therapy of three-dimensional cervical manipulation combined with acupotomy on the treatment of cervical spondylosis radiculopathy and its influence on infrared thermal imaging of the diseased area.Methods:A total of 128 patients with radicular cervical spondylosis meeting the inclusion criteria were selected and divided into four groups according to random number table method,with 32 cases in each group.Group A was the control group,Group B was the three-dimensional cervical manipulation group,Group C was the acupotomy group,and Group D was the three-dimensional cervical manipulation combined with the acupotomy group.All the four groups were treated for three weeks.The changes of Neck Disability Index(NDI)and infrared thermal imaging were compared before and after treatment between the four groups.Results:After treatment and three months after treatment,the NDI scores in Groups B,C and D were compared with those before treatment,differences being significant(P<0.05).After treatment and three months after treatment,the temperature differences in the neck of Groups B,C and D were compared with those before treatment,differences being significant(P<0.05);the temperature difference in the neck of Group D was decreased more significantly than that of Groups B and C,differences being significant(P<0.05).There were no significant differences in the neck temperature difference of Group A after treatment and three months after treatment when compared with that before treatment(P>0.05).Conclusion:The therapy of three-dimensional cervical manipulation combined with acupotomy can improve cervical function,whose clinical effect is better than that of simple three-dimensional cervical manipulation or acupotomy.
